Association between high burn-out and workplace violence among healthcare workers in China: a WeChat-based survey

ObjectivesThis study is conducted to examine whether overall workplace violence (WPV) and its five types are associated with high burn-out among healthcare workers in China.DesignA WeChat-based cross-sectional survey. Snowball sampling was used in this study.ParticipantsFront-line healthcare workers (N=3706) from 149 cities across 23 provinces in China responded to the survey, and 22 questionnaires were excluded because of incomplete data.Primary and secondary outcome measures(1) The Chinese Maslach Burnout Inventory-General Survey was used to measure high burn-out. (2) WPV was assessed using the Chinese version of the Workplace Violence Scale. (3) An anonymous self-designed web-based questionnaire consisting of demographic, behavioural and occupational information was used to identify covariates.ResultsA total of 3684 front-line healthcare workers (934 physicians and 2750 nurses) were included. Of all participants, 13.3% (491/3193) experienced high burn-out. Adjusted logistic regression revealed that experience of WPV in the past year was associated with high burn-out (OR 2.10, 95% CI 1.69 to 2.62). Healthcare workers who had suffered emotional abuse, threat or verbal sexual harassment were more vulnerable to high burn-out.ConclusionThis study finds that healthcare workers with WPV, especially emotional abuse, threat and verbal sexual harassment, are more likely to experience burn-out. These types of WPV should be considered in interventions to reduce and prevent burn-out for healthcare workers.

Introdução: Profissionais da oncologia enfrentam estressores psicossociais no desempenho de suas atividades laborais que os tornam vulneráveis ao Burnout. A Síndrome de Burnout é uma maneira de resposta ao estresse laboral crônico, sendo esta uma condição na qual o trabalhador se desgasta, e tende a desistir, na medida em que perde a satisfação e sentido pelo trabalho. Objetivo: O presente artigo teve como objetivo analisar artigos científicos que abordam a presença da Síndrome de Burnout nos enfermeiros que trabalham na área de oncologia. Metodologia: Trata-se de uma pesquisa de revisão de literatura integrativa realizada com artigos indexados em sites de cunho científico através dos descritores: Síndrome de Burnout, enfermeiros, oncologia. Análise e discussão dos resultados: Através deste estudo, foram constatados que o ambiente hospitalar e o setor de oncologia são locais que produzem grandes cargas de estresse, e fatores predisponentes aos profissionais, principalmente os enfermeiros que estão em contato maior com o paciente, a desenvolverem a Síndrome de Burnout. Conclusão: Conclui-se que existe a necessidade de mais pesquisas sobre a síndrome, principalmente no setor oncológico, ressaltando as condições de trabalho destes profissionais e um foco à saúde física e mental dos trabalhadores.

Introduction: the study of burnout syndrome intensifies over time. In 2020, MEDLINE/PubMed database contains more than 18,000 publications on this phenomenon. In its development, the concept of burnout has passed through several heterogeneous periods. Objective: to provide the description of the origin and development of the burnout concept, the features of the social environment within which this syndrome was actualized, and the current state of this problem.Material and methods: publications on the burnout syndrome were analyzed. The search was performed using the keywords “burnout”, “history” in the MEDLINE/PubMed, eLIBRARY, and Google Scholar databases. The “snowball” method was used to search for publications in the literature lists of relevant studies.Results: the phenomena similar to burnout were registered long before the start of its academic study. The initiation of scientific research on burnout occurred mainly due to global social changes associated with the increasing role of social professions and the increasing burden on workers employed in them. The results of the analysis of publications made it possible to distinguish several stages of burnout studying, which differed in the methodology of empirical research, the range of subjects included in the risk group for this phenomenon, and the number of competing psychological models of burnout. Despite the growing number of papers on burnout, issues related to the clinical status of this syndrome, its discriminant validity, methods of its prevention and coping are still relevant. The effectiveness of an interdisciplinary approach to the study of burnout can be flawed by excessive expansion of the concept and loss of its substantiveness.
