Objectives/Hypothesis Identifying distinctive features of the vocal fold (VF) during development could have significant clinical implications for treating voice disorders. This study investigates the structural organization of the VF microanatomy across gender and age groups using optical coherence tomography (OCT). Study Design Prospective clinical trial. Materials and Methods In vivo OCT images were acquired from 97 patients (58 males and 39 females) aged between 6 weeks and 27 years. All patients showed no signs of vocal fold pathology on endoscopy. Morphological features were extracted from OCT images and statistically compared between age groups. This study was performed at Massachusetts Eye and Ear between 2017 and 2019. Results All OCT acquisitions show a stratified microanatomy across age groups, even in newborns suggesting the presence of a superficial lamina propria (SLP) at birth. Furthermore, the optical scattering in the VF lamina propria changes according to age, suggesting subepithelial maturation. Although the epithelium thickness was relatively constant across age groups, the SLP showed a significant linear relationship between age and thickness (P = .016). Furthermore, a significant difference (P = .002) in SLP thickness was found between young adult males and females. The overall thickness of the entire mucosa did not change significantly with age. Conclusion OCT is a noninvasive imaging modality capable of providing quantitative morphological features to describe the VF development. A stratified structure can be observed in OCT from newborns to young adults. Further investigations could combine OCT, acoustic measurements, and molecular sensitive techniques to provide a complete interpretation of the VF development. Objective When performing lightwand intubation, an improper transmitted glow position before tube advancement can cause intubation failure or laryngeal injury. This study was performed to explore the transmitted glow point corresponding to a priori chosen depth for lightwand intubation. Methods Before lightwand intubation, we marked the transmitted glow point from a bronchoscope on the neck when it reached 1 cm below the vocal cords. Lightwand intubation was then performed using this marking point. The distances from the mark to the upper border of the thyroid cartilage, upper border of the cricoid cartilage, and suprasternal notch were measured. Results In total, 107 patients were enrolled. The success rate of lightwand intubation using the mark was 93.5% (95% confidence interval, 88.7%–99.2%) at the first attempt. The marking point was placed 12.0 mm (95% confidence interval, 10.6–13.4 mm) below the upper border of the cricoid cartilage. Conclusion Anaesthesiologists should be aware of the appropriate point of the transmitted glow on the patient’s neck when performing lightwand intubation. We suggest that this point is approximately 1 cm below the upper border of the cricoid cartilage. Trial registration: ClinicalTrials.gov NCT03480035

Background: The larynx besides being used in respiration, performs the function of production of voice. A thorough knowledge of relevant surgical anatomy is a must for any surgeon before performing laryngeal framework surgery (LFS). Most of the relevant anatomic details for LFS are based on Western population and the studies based on Indian population are extremely scarce. We propose to undertake this study in order to understand the relevant anatomy of the thyroid cartilage in adult Indian population and to compare these dimensions with those studied in the western population as available in literature.Methods: Fifty-two samples of adult thyroid cartilage (35 males and 17 females) taken from cadavers were analysed. Digital Vernier callipers & goniometer were used to take morphometric measurements of the Thyroid cartilage to see for normal variation, sex differences and bilateral asymmetry. Observation were analysed by calculating mean & SD. Results: Most parameters measured in thyroid cartilage did not show significant difference in males & females. All the parameters of thyroid cartilage except thyroid angle were higher in males than females. Bilateral asymmetry in the size of thyroid lamina was seen in majority of specimens though not statistically significant.Conclusions: The detailed study of thyroid cartilage in the form of morphometric parameters of thyroid cartilage is useful for anatomists, radiologists, reconstructive and ENT surgeons. Comparison with other populations in the world reveals that the absolute values differ in different populations which could be due to differences in body built, racial differences and age group of cadavers.
