Diabetes is an untreatable metabolic disorder characterized by alteration in blood sugar homeostasis, with submucosal insulin therapy being the primary treatment option. This route of drug administration is attributed to low patient comfort due to the risk of pain, distress, and local inflammation/infections. Nanoparticles have indeed been suggested as insulin carriers to allow the drug to be administered via less invasive routes other than injection, such as orally or nasally. The organic-based nanoparticles can be derived from various organic materials (for instance, polysaccharides, lipids, and so on) and thus are prevalently used to enhance the physical and chemical consistency of loaded bioactive compounds (drug) and thus their bioavailability. This review presents various forms of organic nanoparticles (for example, chitosan, dextron, gums, nanoemulsion, alginate, and so on) for enhanced hypoglycemic drug delivery relative to traditional therapies.

T2DM was called a non-insulin dependent type. The cause is either insulin resistance with insulin insufficiency or from defect in insulin secretion combined with insulin resistance. It usually occurs before 40 years old age, but also can occur at less than that age. About 50% of male and 70% of female cases often are obese. The management of diabetes has been practiced since the Middle Ages with several treatments. The first treatment called metformin is a biguanide drug that is usually used as the first-line oral treatment for T2DM in people of all ages. The second treatment used is insulin therapy (with or without additional medications) that should be started in newly diagnosed T2DM patients with symptoms (catabolic characteristics such as weight loss, ketosis, or signs associated with hyperglycemia such as frequent urination/polydipsia) and/or have a severe increase in blood sugar levels. Results: In type II diabetic patients who treated with compare of metformin or insulin The results was indicated a significant difference parameters in duration of treatment and blood glucose , as well as , there was a strong positive correlation between ALT and AST in in treated with metformin or insulin. Conclusions: In type II diabetic patients, metformin or insulin treatments have the same effect on the activities of ALT, AST, SOD, GST and lipid profile , except for the level of glucose depending on the disease severity, study the correlation of ALT and AST activities with some biochemical markers in type 2 diabetic patients treated with metformin and insulin. Diabetes mellitus or also called DM is a disease characterized by increased blood glucose levels as a result of the body’s physiological failure to regulate glucose metabolism. The prevalence of people with diabetes mellitus in the world is increasing year by year. Indonesia is ranked with the fourth highest cases of diabetes mellitus in the world. The objective of this study is to identify cases of diabetes mellitus based on gender in the North Buton Regency in the 2017-2020 period. This is a survey research utilizing the health report data of the North Buton Regency Health Office in the 2017-2020 period, which includes data on the DM cases based on gender. The type of research data is numerical. The research data is presented in the form of graphs with narration. The results showed that the number of DM cases in North Buton Regency in 2017 the highest number of DM cases was at the Kulisusu Health Center with 138 cases, in 2018 the highest was at the Waode Buri Health Center with as many as 46 cases, in 2019 the highest DM case number was at the Kulisusu Health Center with as many as 73 cases. In 2020 the highest cases was at the Kulisusu Health Center with as many as 117 cases, where DM cases in women were higher than men. Keywords: Diabetes Mellitus, Gender, Female, Male
