A framework for the economic evaluation of telemedicine

Abstract: This paper describes the economic issues associated with the introduction of telemedicine systems and the main challenges to their evaluation. An approach to the economic evaluation of telemedicine is described based on a cost-consequence framework. The paper links these costs and consequences more formally within a set of evaluative questions which in turn forms the basis for an economic model for evaluating telemedicine. By outlining the key questions, a number of issues relevant to the evaluation of telemed… Show more

“…Furthermore, the evaluation is also dependent on the estimation method used, the characteristics of the technology under assessment, the particularities of the data sources, applicability of the system, applications, and organization and social values where and when the evaluations take place (Mcintosh and Cairns, 1997;Ohinmaa, Hailey and Roine, 1999). This can play an important role in determining the unanticipated consequences of adopting the technology at organizational or national levels.…”

“…This matrix presents the factors that should ideally encompass the economic evaluation. The matrix completes the first analysis of Bashshur [27], McIntosh [28], and Lobley [29]. For the sake of simplicity of use and evaluation feasibility, the proposed framework focusses on economic calculation starting from the information collected on costs rather than on a matrix of indicators.…”

“…A similar analytic framework, which places more emphasis on monetary factors, is the cost-consequence matrix proposed by McIntosh and Cairns. 4 Preliminary studies can generate hypotheses and fruitful areas for further study.…”

“…Decision makers at various levels in health care need more definitive information on the costs and benefits of telemedicine before it can become more widely accepted. [2][3][4][5][6][7][8] Further, economic appraisal of telemedicine must consider the perspectives of the different stakeholders-patients, health professionals, and payers/insurers 9 -as well as the evolution of telemedicine technology and health care systems over time. In this paper we consider the changing requirements for economic evaluation of telemedicine applications, along with the experience in Alberta, Canada.…”
