A Five-Year Comparison of Patellar Tendon versus Four-Strand Hamstring Tendon Autograft for Arthroscopic Reconstruction of the Anterior Cruciate Ligament

Abstract: Arthroscopic reconstruction with either graft results in a similar surgical outcome, reliably restoring knee stability over a 5-year period; however, patients with patellar tendon grafts are at greater risk of developing early signs of osteoarthritis.

“…In fact, Butler et al 12 tested participants an average of 5 years postoperatively, a time when radiographic osteoarthritis may be influencing lower extremity biomechanics. [24][25][26] When comparing KAM in the ACL þ BB and control groups with the ACL-injury group, we observed strong effect sizes with CIs that did not cross zero. These large effect sizes suggest that participants may have had clinically relevant differences in KAM.…”

“…This type of graft has several advantages relative to the common patellar tendon graft, including less anterior knee pain following surgery, less postoperative stiffness, smaller incision, and faster recovery. 7,8 However, it needs to be taken into consideration that studies have also indicated the possibility for decreased hamstring strength in these patients following surgery. 9 Postoperatively, the patient was referred for physical therapy.…”
