A double‐blind study of adjunctive carbamazepine versus placebo on excited states of schizophrenic and schizoaffective disorders

Abstract: A multi-institutional double-blind study comparing the therapeutic effect of adjunctive carbamazepine and placebo with standard neuroleptic treatment was performed on 162 patients with DSM-III diagnosis of schizophrenic (n = 127) or schizoaffective disorders (n = 35) who had excited states or aggressive/violent behavior that responded unsatisfactorily to neuroleptic treatment. The patients participated in a 4-week trial of carbamazepine plus neuroleptics (n = 82) or placebo plus neuroleptics (n = 80). The sum … Show more

“…Anticonvulsant agents are widely prescribed to enhance neuroleptic efficacy (Baldessarini et al 1995;Citrome et al 1998;Wilson et al 1985). However, there are limited supporting data for this application of anticonvulsants in schizophrenic patients (Carpenter et al 1991;Casey et al 2003;Dose et al 1987Dose et al , 1998Greil et al 1997;Klein et al 1984;Nachshoni et al 1994;Okuma et al 1989).…”

“…The evidence for efficacy comes mostly from small trials or case reports, 74-80 but the results of a larger trial (n = 162) are also available. 81 In this latter report, a randomized clinical trial of carbamazepine in patients with DSM-III schizophrenia or schizoaffective disorder, there was a failure to detect significant improvement on the total Brief Psychiatric Rating Scale (BPRS). Differences did emerge, however, on the items of suspiciousness, uncooperativeness, and excitement.…”
