A Diagnostic Marker to Discriminate Childhood Apraxia of Speech From Speech Delay: I. Development and Description of the Pause Marker

Abstract: Purpose The goal of this article (PM I) is to describe the rationale for and development of the Pause Marker (PM), a single-sign diagnostic marker proposed to discriminate early or persistent childhood apraxia of speech from speech delay. Method The authors describe and prioritize 7 criteria with which to evaluate the research and clinical utility of a diagnostic marker for childhood apraxia of speech, including evaluation of the present proposal. An ov… Show more

“…Severity findings for the 4 classifications and retrospective longitudinal findings from 8 participants with CAS supported the validity and stability of the PMI. Conclusion: Findings support research and clinical use of the PMI to scale the severity of CAS.A four-article series (Shriberg et al, 2017a(Shriberg et al, , 2017b(Shriberg et al, , 2017c, and the present article) to develop and assess the theoretical and clinical utility of a singlesign, behavioral marker of childhood apraxia of speech (CAS) poses the following three questions: …”

“…It is useful to reproduce Figure 1 from the first article in this series (PM I; Shriberg et al, 2017a), which will be referenced throughout the present article. Figure 1, the research framework for the PM termed the Speech Disorders Classification System (SDCS), includes a sevenelement generic representation of speech processing stages.…”

“…Figure 1, the research framework for the PM termed the Speech Disorders Classification System (SDCS), includes a sevenelement generic representation of speech processing stages. Recall (PM I; Shriberg et al, 2017a) that Figure 1 is the finalized version of the SDCS with the exception of some updates at Level III and Level IV that are not pertinent in the present context (Shriberg, Strand, & Mabie, 2017). The following sections describe relevant terms and concepts in CAS research, followed by brief literature overviews of research addressing two hypotheses about speech processing deficits in CAS.…”

“…T he first article in this series (PM I; Shriberg et al, 2017a) includes a review of the development of a diagnostic marker of childhood apraxia of speech (CAS) termed the Pause Marker (PM). The goal was to develop a procedure that maximally meets the seven attributes of a highly valued diagnostic marker described and prioritized in that article's Table 1.…”
