A De Garengeot Hernia masquerading as a strangulated femoral hernia

Abstract: INTRODUCTIONDe Garengeot hernia is a rare occurrence whereby an appendix is found in a femoral hernia sac. It is rarer still to find an acutely inflamed appendix manifesting itself as a strangulated femoral hernia. This case is important to report as it highlights the diagnostic difficulty this particular condition presents to an emergency surgeon.PRESENTATION OF CASEWe report the case of an 86 year old female who was found to have a De Garengeot hernia containing a necrotic appendix. A retrograde appendicecto… Show more

“…Some suggest that intraluminal obstruction initiates inflammatory changes in the appendix leading to subsequent migration of the appendix into the hernia sac. However, others suggest migration of the appendix is the initial event, and the narrow rigid neck of the femoral sac causes incarceration and strangulation of the appendix 8. This seems consistent with our patient because no obvious intraluminal obstruction was noted and haemorrhage in the periappendiceal soft tissue was present.…”

“…Laparoscopic appendectomy and hernioplasty via anterior approach is recommended in some cases in order to prevent surgical site infection and mesh contamination . Simple suture hernioplasty instead of mesh repair is suggested by a number of authors for the same reason . In our case, we performed the appendectomy and the mesh hernioplasty through the same incision without any signs of surgical site contamination.…”

“…Clinical examination is of limited value in identifying the content of the femoral hernia. In all of the reviewed case reports, including our case, ultrasound has not been reported as able to diagnose a de Garengeot's hernia [2, 15–18]. Depending on protocol and reporting expertise, CT has demonstrated some value, with only 4 reported cases where CT has given the correct diagnosis [17, 1921].…”

“…This phenomenon is thought to occur in 0.5% to 3% of femoral hernia cases, with a female : male ratio of around 6 : 1 [2, 3]. …”
