Aims and Objectives The aim of this integrative review was to synthesise empirical reports of interprofessional collaborative practice (IPCP) for community‐dwelling older adults and uncover barriers and facilitators related to its success as a model of care for this population. Background IPCP is a model of care that has demonstrated positive outcomes for community‐dwelling older adults. However, a summary of barriers and facilitators to IPCP models has not been presented. Methods An integrative review using the method posited by Whittemore and Knafl was completed to identify barriers and facilitators to IPCP for community‐dwelling older adults. The literature search was reported following PRISMA guidelines. Results Four themes emerged as barriers to IPCP: (1) A (Potential) Logistical Nightmare, (2) All About the Money, (3) If We Can't Test It, Can We Recommend It? and (4) Challenging for the Team, Challenging for the Client. Three themes emerged as facilitators to IPCP: (1) Reducing Resource Waste, (2) The “C” in IPCP and (3) What Matters Most. Conclusions IPCP models for community‐dwelling older adults must adapt to the setting of care and client needs. Interprofessional education opportunities for team members facilitate effective IPCP. Healthcare policies and funding structures need to address IPCP for community‐dwelling older adults for this model to be successful and sustainable. Relevance to Clinical Practice Nurses participate on and lead IPCP teams caring for community‐dwelling older adults and, therefore, need to be aware of barriers and facilitators to this model of care.

Systems of communication in school‐nurse led care coordination: A concept analysis

Aim: To analyze the concept of systems of communication in school nurse-led care coordination to develop an operational definition that will inform intervention development. Background: Communication has been identified as an essential attribute in care coordination. However, previous concept analyses of care coordination did not clearly define systems of communication or consider the context of school-based care coordination. Defining and conceptualizing systems of communication has important implications for improving school nurse-led care coordination. Methods: Concept analysis was conducted using Walker and Avant's eight-step concept analysis method. The literature was searched to identify supporting literature that was analyzed using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses checklist. Results: Systems of communication in school nurse-led care coordination can be defined as systems wherein care team members, led by the school nurse, collaborate by communicating information and knowledge through an individualized healthcare plan that is student/family-centered and shared through information systems. Attributes require developing an individual health plan that incorporates care coordination needs, information sharing with student/ family consent, and a clear delineation of team member roles. Consequences include student/family outcomes, team member knowledge, and efficiency and accuracy of information. Conclusions: Concept clarification and a synthesized definition allow for more effective measurement development for effective communication in school nurse-led care coordination. Students with healthcare needs in the school environment require systems of communication that efficiently work toward school nurse-led care coordination that addresses the student's health and academic outcomes.

The purpose of this study was to obtain empirical evidence of the role of work motivation in mediating the relationship between interprofessional collaboration and transformational leadership on patient safety in inpatient installations using a cross-sectional study design. Moreover, make 250 inpatient installation nurses with civil servant status as the unit of analysis. The study's results prove that interprofessional collaboration and transformational leadership positively and significantly affect work motivation and patient safety. Work motivation has a positive and significant direct effect on patient safety, and work motivation has a positive role in mediating interprofessional collaboration and transformational leadership on patient safety. The importance of patient safety culture will be of value to hospital performance in patients' eyes so that maximum results are formed on implementing patient safety culture. Not only talking about the effectiveness of interprofessional collaboration and transformational leadership of the head of the room but the importance of implementing work motivation in the form of encouragement to nurses. Will create satisfaction that impacts the effectiveness of interprofessional collaboration and transformational leadership in improving patient safety culture in inpatient installations.
