A Comparison of Lignocaine 2% with Adrenaline 1:200,000 and Lignocaine 2% with Adrenaline 1:200,000 plus Fentanyl as Agents for Caudal Anaesthesia in Children Undergoing Circumcision

Abstract: This randomised double-blind study compares the onset, quality and duration of analgesia in two groups of children (n = 30), aged between two and twelve years undergoing circumcision. Under concurrent general anaesthesia, the children received caudal anaesthesia employing either lignocaine 2% with adrenaline 1:200,000 plus normal saline 0.01 mllkg, or lignocaine 2% with adrenaline 1:200,000 plus fentanyl 1.0 microgramlkg. The results show no statistical or clinical difference in the onset, duration, quality of… Show more

“…Respiratory depression is an expected but unwanted side effect of extradural opioid;[31] it has also been noticed in adult patients who received clonidine 300 μg extradurally. [32] Many previous studies have however not reported respiratory depression after caudal administration of fentanyl[252633] or clonidine[27283034] in pediatric patients. In our study, a transient decrease of oxygen saturation to 91% was observed in 5 cases of group II, while no patient of group I suffered hemoglobin desaturation ( P = 0.0105).…”

“…However, Jones et al. (8) showed that the duration of analgesia was 157 and 185 min in group lignocaine 2% with epinephrine (1 : 200 000) plus fentanyl and in group lignocaine 2% with epinephrine 1 : 200 000, respectively. In our study, the duration of analgesia was found to be 1206.09 ± 377 min.…”

“…There was a significant difference between the groups with respect to this incidence ( P <0.001). The bupivacaine 0.125% group had significantly more patients who received fentanyl ( 11) than each of the 0.25% groups (two in each group). The 0.125% bupivacaine with fentanyl group (six patients) did not differ from either of the 0.25% bupivacaine groups with respect to intravenous fentanyl administration.…”
