A cluster of acute hepatitis C virus infection among men who have sex with men – results from contact tracing and public health implications

Abstract: A cluster of acute HCV infection is reported among mostly HIV-positive MSM, with multiple partners throughout Europe. Sexual techniques potentially leading to mucosal damage (fisting), concomitant STDs such as LGV and drug use seem facilitating factors for spread. Extensive case finding and partner tracing is advocated as well as targeted prevention messages.

Halfon dkk 44 melaporkan sebuah kasus hepatitis C akut yang disebabkan oleh transmisi seksual pada pasangan heteroseksual setelah hubungan seksual anal dan vaginal yang dibuktikan dengan analisis filogenetik virus hepatitis C. Ghosn dkk 23 melaporkan serial kasus hepatitis C akut pada kelompok homoseksual dengan HIV positif dan mendapatkan adanya hubungan antara perilaku seksual risiko tinggi termasuk hubungan anal yang tidak dilindungi, bersamaan dengan infeksi sifilis dan transmisi hepatitis C. Sedangkan Gotz 24 melaporkan pada sebuah kluster infeksi hepatitis C akut pada kelompok homoseksual dengan HIV positif didapatkan faktor yang berhubungan dengan hepatitis C akut adalah teknik seksual yang merusak mukosa (fisting) dan infeksi menular seksual LGV. Data-data di atas dapat mendukung hipotesis bahwa risiko transmisi seksual hepatitis C dapat meningkat jika terjadi kerusakan barrier mukosa anorektal yang memfasilitasi masuknya virus hepatitis C melalui perlukaan, selain keberadaan HCV RNA dalam semen atau pun dalam darah yang tidak diperiksa pada penelitian ini.…”
