Objective: To find out the frequency and types of malignancy in patients treated surgically for nontoxicmultinodular goiter and its correlation in different age groups and gender. Study Design: Cross-sectional study. Place and Duration of Study: Department of ENT, Combined Military Hospital Rawalpindi, from Dec 2018 toDec 2019. Methodology: A total of 116 patients operated for nontoxic multinodular goiter were included in the study.Demographic details along with pre-op Ultrasonography findings, Fine Needle Aspiration Cytology, thyroidfunction test results and post-op histopathological reports of the thyroid specimen were entered on pre-designed proforma. Results were analyzed using the Statistical Package for Social Sciences (SPSS) Version 22. Results: Age ranges from 19 to 70 years (mean 43.51 years). Out of 116 cases operated for multinodular goiter,37 (31.8%) had malignant thyroid lesions (13 males, 24 females). Papillary carcinoma of the thyroid (45.9%) wasthe commonest malignancy followed by a follicular variant of papillary carcinoma (24.3%). The most common age group with malignancies was 49-58 years. Conclusion: Malignant Thyroid cancers showed a female preponderance and were most common in age group49-58 years. Papillary carcinoma of thyroid was the commonest tumor. Radical thyroid surgery is a recommended surgical management option for nontoxic Multinodular goiter.

Background: Enlargement of thyroid gland is the most common manifestation of thyroid disease. Most of the patients come from endemic zones. This study was designed to study various aspects of clinicopathological presentations of multinodular goitre and their optimal surgical management. Methods: This was a prospective observational study of 50 cases of multinodular goitre with an adequate follow up to look for epidemiological parameters such as clinical presentation, pathological behaviour and modality of management including post operative complications over a period of 02 years. Results: Females were predominant in number over males with a sex ratio 24:1. FNAC was done for all patients and was found to be 100% accurate. Majority of the patients were treated with total thyroidectomy (56%), followed by hemithyroidectomy (44%). Complications included wound infection (4%) and seroma formation (4%). Conclusions: Multi nodular goitre is more common in females and majority are in the age group of 31-39 years. The chief complaint in majority of the patients is swelling in front of the neck and few patients with pressure symptoms. FNAC is an invaluable, minimally invasive, highly accurate and cost-effective procedure for preoperative diagnosis and management of multinodular goitre. The main indication for surgery in MNG is cosmetic problem and total thyroidectomy is the ideal treatment of MNG with minimal complications.

Thyroid disorders are associated with the abnormalities occur in thyroid hormone concentration and imbalance in their regulation may causes life threatening diseases. Thus, the present study was planned to establish the prevalence of thyroid defects in Diyala, Iraq due to paucity of information is available in our region. Total 67 subjects in the age range of 20-80 years were recruited in Al -Shafa hospital, Diyala, Iraq, suffering from thyroid disorders or suspected with the symptoms favoring thyroid diseases. Thyroid function test and physical examination were performed. Those who were found alteration in thyroid hormones and presence of nodules, further investigation like FNAC of neck was performed for the diagnosis. Once confirmation of thyroid defect, total thyroidectomy was performed as a first line of treatment and malignancy was confirmed using histopathology analysis. The incidence of thyroid diseases in female (86.57%) was found to be more than male (13.43%) and there ratio was 19.3:3. The thyroid defects incident rate was highest in mean 49 years age. Almost 61.2% patients were found affected by thyroid disorders in this age group. The largest number of patients (64.18%) had shown MNG, then other thyroid defects out of which 4 female patients presented very big thyroid multinodular goiter (MNG). Among 43 MNG patients, 86% patients were female and 4% were male. Of the 67 cytologically diagnosed cases, 79.10% cases were found benign cases which are highest among all other types, out of which 91% cases were female. Among the thyroid disorder burden of thyroid carcinoma is very high and common in the enrolled patients in Diyala, further longitudinal studies are required to explore the prognosis and causes of these thyroid defects in the Iraq.
