A clinico-dermatoscopic study of 100 cases of melasma in a tertiary care hospital

Abstract:
Background: Melasma is one of the most common hyperpigmentary disorders seen among Indian patients. Dermatoscope is a non-invasive tool that helps in visualization of surface and sub-surface changes. Recognition of characteristic dermatoscopic patterns of melasma helps in differentiating it from other hyperpigmentary disorders. The present study was carried out to study clinical and dermatoscopic patterns of melasma.

“…Hence, it can help in diagnosis, prognosis, and monitoring the therapeutic efficacy of melasma. 15 In this study, the percentage of improvement in TDS was higher in the epidermal type than other types of melasma in both sides. This difference was only significant in the combined side.…”

“…Melasma lesions were examined thoroughly for detailed pigment patterns and vascular features 15 including accentuated pseudoreticular network, reticuloglobular pattern, dotted pattern, arcuate pattern, perifollicular involvement, and visible telangiectasia. A proposed score was given to each individual variable where "not detected 5 0", "hardly detected 5 1", "detected 5 2", and "obvious 5 3".…”

“…Though melasma does not cause any major health related problems it has a deleterious impact on the health-related quality of life. It severely affects social life, emotional well-being and physical health of the patients [8]. Females are more commonly affected [9] than males.…”

“…MDA is the stress marker that indicates lipid per-oxidation of endogenous lipid has been shown to be a major risk factor leading to myocardial injury in metabolic syndrome (38). When myocardial cells are damaged, leakage of myocardial enzymes such as LDH and CK-MB occurs which accounts for the increased activities of serum biomarker enzymes and therefore, serve as a sensitive index to assess the severity of irreversible myocardial injury LDH converts pyruvate, the final product of glycolysis to lactate when oxygen is absent or in short supply and the activity of LDH will increase when ischemia lasts (39). In un-treated fructose-fed IR rats markedly affect hemodynamic parameters, could be due to loss of normal heart muscle architecture, abnormal cardiac physiology and the loss of glycogen (40).Consumption of fructose-enriched diet in rats able to increases oxidative stress level in heart, and also the initiation of tissue injury were noted necrosis, loss of normal heart muscle architecture and also reported cardiac injury markers in serum.…”
