A case of post-traumatic LASH syndrome responsive to indomethacin and melatonin (a man with a unique triad of indomethacin-responsive trigeminal autonomic cephalalgias)

Abstract: This is the first post-traumatic/secondary case of LASH syndrome, the first male patient to be documented with LASH, and the first LASH case to show complete alleviation with melatonin. This patient's unique case history provides another example of how multiple trigeminal autonomic cephalalgias can occur in a single individual. This may be one of the first cases of three distinct trigeminal autonomic cephalalgias developing after trauma.

“…78 Interestingly, melatonin achieved complete pain alleviation in the first post-traumatic/secondary case of long-lasting autonomic symptoms with hemicrania (LASH) syndrome. 80 Moreover, exogenous melatonin supplementation can significantly relieve abdominal pain caused by irritable bowel syndrome (IBS). 81 Furthermore, melatonin reduces indomethacin dosage during the treatment period of hemicrania continua and shows better pain relief effect.…”

“…Indomethacin-responsive headaches have been claimed to be good candidates for melatonin supplementation (112), since both share similar chemical structures (113). Case reports have been published showing melatonin efficacy in hemicrania continua (114,115), paroxysmal hemicranias (116), LASH syndrome (117) idiopathic stabbing headache (118,119), and hypnic headaches (53).…”
