Choledochoduodenal fistula (CDF) is an abnormal communication between the common bile duct and the duodenum. It accounts for about 5% to 25% of the total biliary fistulas and is usually due to a perforated duodenal ulcer, choledocholithiasis, and complications secondary to tuberculosis or could be iatrogenic. Primary intrabilliary tumors usually cause obstructive jaundice and rarely biliary metastasis arising from other organs like colon, breast, and lungs can cause obstructive jaundice. There has been a case report of metastasis from ovarian cancer to the major papilla of the duodenum but no reported cases of it causing a CDF. We report a rare case of an 83-year-old female with ovarian cancer who developed a metastatic lesion to the duodenum eventually resulting in a CDF.

Malignant biliary obstruction generally results from primary malignancies of the pancreatic head, bile duct, gallbladder, liver, and ampulla of Vater. Metastatic lesions from other primaries to these organs or nearby lymph nodes are rarer causes of biliary obstruction. The most common primaries include renal cancer, lung cancer, gastric cancer, colorectal cancer, breast cancer, lymphoma, and melanoma. They may be difficult to differentiate from primary hepato-pancreato-biliary cancer based on imaging studies, or even on biopsy. There is also no consensus on the optimal method of treatment, including the feasibility and effectiveness of endoscopic intervention or surgery. A thorough review of the literature on pancreato-biliary metastases and malignant biliary obstruction due to metastatic non-hepato-pancreato-biliary cancer is presented. The diagnostic modality and clinical characteristics may differ significantly depending on the type of primary cancer. Different primaries also cause malignant biliary obstruction in different ways, including direct invasion, pancreatic or biliary metastasis, hilar lymph node metastasis, liver metastasis, and peritoneal carcinomatosis. Metastasectomy may hold promise for some types of pancreato-biliary metastases. This review aims to elucidate the current knowledge in this area, which has received sparse attention in the past. The aging population, advances in diagnostic imaging, and improved treatment options may lead to an increase in these rare occurrences going forward.

Latar Belakang : Ikterus atau jaundice adalah perubahan warna jaringan menjadi kekuningan akibat pengendapan bilirubin. Tumor merupakan salah satu penyebab ikterus obstruktif yang paling sering. Beberapa penelitian sebelumnya menunjukkan gambaran klinis ikterus obstruktif yang disebabkan tumor berdasarkan umur, jenis kelamin, tatalaksana, gambaran CT Scan, Histopatologi. Tujuan Penelitian : Tujuan penelitian ini untuk mengetahui gambaran klinis ikterus obstruktif yang disebabkan tumor di RSUD Dr. H. Abdul Moeloek Bandar Lampung Periode 2017. Metode Penelitian : Jenis penelitian ini adalah deskriptif retrospektif menggunakan data sekunder dari rekam medik. Sampel penelitian ini menggunakan teknik Total Sampling. Hasil Penelitian : Sampel penelitian ini sebanyak 60 pasien. Frekuensi terbanyak ditemukan pada kelompok umur 50-59 tahun sebanyak 15 pasien (25%) dan 32 pasien (53.3%) pada laki-laki. Penatalaksanaan dilakukan tindakan operatif sebanyak 51.7%. Dari hasil pemeriksaan CT Scan didapatkan pada hati sebanyak 51.7% dan dari hasil histopatologi didapatkan Hepatocellular Ca sebanyak 26.7%. Kesimpulan : Gambaran klinis terbanyak ditemukan pada kelompok umur 50-59 tahun, jenis kelamin laki-laki dengan dilakukan tindakan operatif, gambaran CT Scan pelebaran intrahepatic atau ekstrahepatic pada hati, histopatologi Hepatocellular Ca.
