A Case of Lobular Capillary Hemangioma (Pyogenic Granuloma), Localized to the Subcutaneous Tissue, and a Review of the Literature

Abstract: Lobular capillary hemangioma (pyogenic granuloma) is a common vascular proliferation that typically occurs in the superficial dermis, although rarely a subcutaneous form has been reported. Although these lesions are considered benign, localized recurrence after excision and satellite spread of the lesions are known phenomena. A case of lobular capillary hemangioma with the unusual features of both subcutaneous localization and locally aggressive behavior following surgery in a patient with a history of estroge… Show more

“…The vascular proliferation in pyogenic granuloma mainly involves capillaries in a lobular arrangement. 12 The vessels in angioplasmocellular hyperplasia, on the other hand, are scattered or loosely grouped, involving both capillaries and venules. That, plus the striking predominance of plasma cells in our patients' lesions, exclude the diagnosis of pyogenic granuloma.…”

“…1,2 The name ''pyogenic granuloma'' is not correct for LCH because the condition is not associated with pyogenic infection and it does not histologically represent a granulomatous inflammation. Therefore, the term ''lobular capillary haemangioma'' was suggested to better describe the characteristic histological features of this lesion.…”

“…The capillaries vary from being tightly packed with scant lumens to larger dilated vascular spaces. [1][2][3][4][5][6][7][8] Histologically, LCH is made up of two main areas: a lobular and a superficial ulcerative area. The lobular area shows a capillary proliferation with a microscopically distinct lobular architecture, and the superficial ulcerative area shows superficial neutrophilic infiltrates and irregular dilatation of blood vessels.…”

“…This lesion is commonly found in areas such as the lips, tongue and buccal mucosa. Pyogenic granuloma may vary considerably in size and may occur at any age (18). There are several treatment options, including surgical excision, electrodessication, curettage/shave excision, lasers, sclerotherapy, imiquimod cream and cryosurgery.…”
