A Case of Erythroplasia of Queyrat Treated with Imiquimod 5% Cream and Excision

Abstract: Imiquimod is a new immunomodulating agent with antitumor and antiviral properties that has been shown to be clinically effective in various kinds of skin diseases, including precancerous dermatoses. Erythroplasia of Queyrat is a carcinoma in situ that mainly occurs on the glans penis. There are several non-invasive treatment options for erythroplasia of Queyrat such as photodynamic therapy, cryosurgery and applying various kinds of topical agents. We now report a case of typical erythroplasia of Queyrat on gla… Show more

“…Imiquimod, the only immune response modifier approved for clinical applications (as Aldara), has been successfully used to treat actinic keratosis and superficial basal cell carcinoma 7–9. In addition, the off‐label use of imiquimod cream has been effective in treating precancerous lesions and malignancies of several mucosal surfaces, including neoplasm of the vulvar epithelium, lip ulcers, erythroplasia of Queyrat, melanoma of the intraepithelial oral mucosa, and even oral leukoplakia 9–13. However, all of these procedures used the original (Aldara) cream to treat lesions in the oral cavity.…”

“…In our case study, topical therapy with imiquimod 5% effectively treated the patient's EQ as evidenced clinically and by follow-up OCT. Several other case reports have also demonstrated the successful treatment of EQ with imiquimod 5% both in the presence and absence of HPV type 16 infection [3,16,17,18,19]. The results of these case reports together suggest that both the antiviral and antitumour effects of imiquimod are important in its ability to treat EQ.…”

“…The etiology of EQ is linked to HPV infection (8,16,39 and 51 subtypes), but risk factors have also been described (social and cultural habits, hygienic procedures, religious practices, heat, friction, trauma and co-infection with human papillomavirus type 8. [7][8][9] EQ is characterized clinically by the presence of unique or multiple well-demarcated, velvety, shiny, intense red plaques on the glans penis, the foreskin, or the urethral meatus of elderly male patients. 7 These are clinical features in favor of EQ.…”
