A case of De Garengeot’s hernia with acute appendicitis and literature review

Abstract: HighlightsDe Garengeot hernia is defined as femoral hernia containing the appendix.De Garengeot hernia with appendicitis is a very rare surgical finding.Diagnosis most often is made intraoperatively.Treatment depends on complications of the hernia and the anatomical position of the appendix.Prompt surgical treatment is essential in order to avoid severe complications.

“…Femoral hernias constitute an uncommon subtype of groin hernias, representing around 3% of all abdominal wall hernias, and usually appear in elder women [5]. Because of the narrow wall defect, femoral hernias have an increased risk of incarceration.…”

“…It is clear that the use of prosthetic mesh is safe in the absence of local complications, such as abscess and local perforation. However, if these or other risk factors for surgical site infection are present, an anatomic repair or an appendectomy followed by interval hernioplasty must be considered [5]. In the event of complications, the laparoscopic approach is a valid option, as it makes the isolation of the mesh from the infected site feasible, decreasing the risk of surgical site infection.…”

“…The incidence of De Garengeot’s hernia cited in the literature is fewer than 100 cases [ 5 ]. De Garengeot’s hernia is diagnosed most times randomly during surgery and should be distinguished from Amyand hernia, which is defined as inguinal hernia containing the appendix and is a more frequent surgical finding due to higher prevalence of inguinal hernias [ 6 ]. There is little standardization in the literature regarding management, however, as was performed in our case, most authors agree that an appendicectomy and hernia repair is the standard treatment [ 7 ].…”

“…Femoral hernias occur as a weakness and protrusion of abdominal viscera or contents through the femoral canal, bordered superiorly by the inguinal ligament and laterally by the femoral vessels. The femoral hernia is much more common in women and accounts for 3% of all hernias [ 1 ]. Due to its narrow neck, it has a much larger chance of strangulation compared to inguinal hernias and all are recommended for repair [ 1 , 2 ].…”

The migration of the appendix into the inguinal hernia sac is known as an Amyand hernia and is very rare.…”
