Purpose Though a static compliance over the entire filling phase of the bladder can be easily assayed from the cystometry, a protocol measuring the dynamic compliance at a specific filling stage has not been established in human subjects/patients. As objective benefits of robotic-assisted sacrocolpopexy (RSCP) to the bladder function, particularly urine storage, of pelvic organ prolapse (POP) patients wait to be established, this study investigated the impact of RSCP on the bladder storage by focusing on its impact on compliance using the pressure-volume analysis (PVA), a protocol used to measure compliance dynamics in animals. Materials and Methods: Pre- and post-operative PVAs derived from urodynamic studies of a total of 27 female POP patients, who underwent RSCP for prolapse stage ≥ II (POP Quantification system) were retrospectively reviewed. Bladder compliance, infused volume, and threshold pressure were analyzed. Results: Compared with the pre-operative control, RSCP increased the mean compliance of the filling phase (p<0.01, N=27). Notably, RSCP increased the compliance of the late half (p<0.01, N=27) but displayed a negligible effect on that of the early half (p>0.05, N=27) of the filling phase. Mechanistically, RSCP decreased the change of detrusor pressure (p<0.01, N=27) without significantly affecting that of infused volume (p>0.05, N=27). Furthermore, RSCP decreased the ΔPdet in the late half (p<0.05, N=27) but not that in the early half (p>0.05, N=27) of the filling phase. Conclusions: RSCP improved bladder storage as it increased bladder compliance, particularly the late filling phase; possibly by restoring anatomical location and geometric conformation for bladder expansion.

Objectives: This study is the first to urodynamically and histopathologically evaluates the effects of bladder diverticulum (BD) secondary to bladder outlet obstruction (BOO). Methods: Guinea pigs (n=32) weighing 900–1,000 g were divided randomly into four groups: Sham, BD, BOO, and BD combined with BOO. All guinea pigs in the four groups underwent urodynamic evaluation preoperatively and at 1 month postoperatively. The bladders were removed and examined histopathologically. Results: The post-operative filling detrusor pressure was lower in the Sham group (7.1±1.6 cm H 2 O) than in the BD (21.4±5.6 cm H 2 O) and BD with BOO groups (23.6±9.3 cm H 2 O) (p<0.05). There was no difference between the Sham and BOO (9.5±4.0) groups. Post-operative bladder compliance was better in the Sham group (2.3±0.8 ml/cm H 2 O) than in the BD (0.9±0.22 ml/cm H 2 O) and BD with BOO groups (0.6±0.3 ml/cm H 2 O) (p<0.05). Involuntary detrusor contraction was not observed in the Sham or BOO groups, but was observed in 37.5% of subjects in the BD and BD with BOO groups. On histological examination, the bladder wall was thicker (3.75±0.68 mm) (p=0.601), and the connective tissue volume was significant increased (p=0.046), in the bladder muscularis mucosa in the BD groups compared to the BOO group. Conclusion: Physiological and histopathological changes in the bladder with BD combined with BOO are more evident than with BOO alone.

Introduction:The urinary bladder diverticula (BD) secondary to benign prostatic hyperplasia (BPH) is a complication that can lead to urinary stasis, stone, urinary tract infection (UTI) and tumors. It's role in acute urinary retention (AUR) is not totally understood.Objectives:To determine the effect of BD size on AUR rates in patients with BPH candidates to surgery.Subjects and Methods:We performed a retrospective cohort study of 47 patients with BPH and BD who underwent BPH surgery associated to complete bladder diverticulectomy from 2006 to 2016. We analyzed risk factors for AUR in patients with BD using univariate, multivariate and correlation analysis.Results:There was a difference in the size of the diverticula, with 6.8 cm vs. 4.5 cm among patients with and without AUR respectively (p=0.005). The ROC curve showed a correlation between the size of BD and the risk of AUR. The value of 5.15 cm presented a sensitivity of 73% and a specificity of 72%. The area under the curve was 0.75 (p=0.01). Comparing groups with BD >5.0 cm vs. ≤5.0 cm, the AUR incidence was 74% and 27.8% respectively with an OR of 2.65 (1.20-5.85) (p=0.005). In the multivariate analysis, only the size of the diverticula reached statistical significance (p=0.012).Conclusions:The diameter of BD is an independent risk factor for AUR in patients with BPH and BD who are candidates to surgery. A diameter greater than 5.15 cm increases the risk of AUR.
