A Basis for Prospective Payment for Home Care

Abstract: In this pilot stu4, a production function model was used as a framework to explore the relationships between Medicare clients' living arrangements, functional status, plan .f care and consumption of home care resources. T h e j n dings showed that the independent variables significantly explained resource consumption and that dtyerences appear to exist for age and gender cohorts. These results provide a beginning direction for dejining a basis for setting prospective reimbursement rates.

“…The Omaha System also has been used to evaluate home care practice or predict resources or outcomes in home care. 19-23 One of the findings was that an increase in the number of nursing problems was associated with higher resource use. 19, 22 Helberg’s study 21 also supported the relationship between increased nursing interventions and resource utilization.…”

Although sociodemographic factors may not directly influence use of home care, they are risk factors for disease develop-nursing visits and agency reimbursement under Medicare conditions of participation.…”

“…They include the primary medical diagnosis, comorbidities and complications arising from the illness or medical care, and whether or not the patient had a surgical procedure before home care admission. The majority of patients who use these services have more than one medical diagnosis and chronic health problems, and are treated for cardiac, respiratory, or musculoskeletal conditions (Harris et al, 1987;Pasquale, 1987Pasquale, , 1988Ruther & Helbing, 1988). Patients with cancer required more home care nursing visits than those with cardiac disease, and number of diagnoses was a significant predictor of nursing visits per day for the former (Ballard & McNamara, 1983).…”
