Low-Cost, Smartphone-Based Specular Imaging and Automated Analysis of the Corneal Endothelium

Abstract: Purpose Specular and confocal microscopes are important tools to monitor the health of the corneal endothelium (CE), but their high costs significantly limit accessibility in low-resource environments. We developed and validated a low-cost, fully automated method to quantitatively evaluate the CE using smartphone-based specular microscopy. Methods A OnePlus 7 Pro smartphone attached to a Topcon SL-D701 slit-lamp was used to image the central corneal endothelium of 30 ey… Show more

“…Enfin, relevons que la mosaïque endothéliale cornéenne peut être révélée et photographiée facilement en utilisant la lampe à fente avec un smartphone monté dessus par réflexion spéculaire. Cette dernière utilisation présente un intérêt diagnostique et didactique non négligeable [10].…”

“…Smartphone-based specular microscopy is expected to be applied for screening abnormalities of corneal endothelium in rural and underdeveloped countries [ 51 ]. In 2021, Mantena et al [ 52 ] developed an image processing method for smartphone-based specular microscopy using a smartphone attached to a digital slit lamp that allowed for automated computation of ECD, CV, and hexagonality values, which achieved good concordance with the values evaluated with conventional specular microscopy.…”

“…In recent years, smartphone-based innovations have shown potential to expand access to ophthalmological screenings due to their inexpensive and scalable nature. For instance, a low-cost, point-of-care tool for evaluating corneal endothelial health has demonstrated comparable accuracy to "gold standard" specular microscopy, which costs tens of thousands of dollars 7 . Another smartphone-based device uses an illuminated 3D-printed attachment and an image analysis pipeline to diagnose keratoconus 8 .…”
