Aims To report a series of benign ovarian seromucinous neoplasms, an uncommon and hitherto poorly described category of tumours included in the current 2014 World Health Organisation classification of tumours of the female reproductive organs. Methods and results We report the clinicopathological features of a series of 22 benign ovarian seromucinous neoplasms (cystadenomas and adenofibromas or admixtures). The neoplasms occurred in patients aged 32–83 years (mean = 62, median = 65.5) and involved the left ovary (n = 14), the right ovary (n = 6) or both ovaries (n = 2). There was a common association with endometrioid elements (endometrioid differentiation within the cystadenoma/adenofibroma and/or endometriosis) and other endometriosis‐associated neoplasms. Conclusions We speculate that some of these represent benign ovarian endometrioid neoplasms with foci of mucinous and/or serous differentiation, while others represent true mixed neoplasms.

Introduction: Magnetic resonance (MR) relaxometry provides a noninvasive predictive tool to discriminate between benign ovarian endometrioma (OE) and endometriosis-associated ovarian cancer (EAOC). Transverse relaxation rate R2 value was determined using a single-voxel, multi-echo MR sequence (HISTO) by a 3T-MR system. R2 with cutoff value of 12.1 s −1 was established to discriminate between benign and malignant tumors. Patient concerns: We present a case of a 39-year-old woman who was initially thought to be malignant transformation of endometriosis by diagnostic MR imaging of the vascularized solid components. Diagnosis: A R2 value of 42.62 s −1 on MR relaxometry demonstrated that this case is non-malignant. Interventions: To confirm the diagnose, left salpingo-oophorectomy by laparoscopic surgery was performed. Outcomes: Histopathological results revealed seromucinous borderline tumor (SMBT). Our experience suggests that preoperative MR relaxometry may be useful for discriminating “borderline (SMBT)” from “malignancy (EAOC).” Furthermore, immunohistochemical studies of this case demonstrated ovarian SMBT cells were positive for estrogen receptor, progesterone receptor, and hepatocyte nuclear factor-1beta. A similar expression pattern was also observed in patients with benign OE. Lessons: In many respects, SMBT characteristics differ from those of EAOC but resemble those of benign OE. MR relaxometry unveils a new clinical approach as an adjunctive modality for discriminating SMBT from EAOC.

Background: In view of the similar MRI ndings of endometriosis-related ovarian neoplasms, this study aimed to investigate the feasibility of whole-tumor apparent diffusion coe cient (ADC) histogram analysis for differentiating endometriosis-related tumors: seromucinous borderline tumor (SMBT), clear cell carcinoma (CCC) and endometrioid carcinoma (EC).Methods: A total of 85 patients (22 with SMBT, 42 with CCC and 21 with EC) were retrospectively enrolled. Clinical data including age, stage, laterality, solid component ADC (ADC SC ) and whole-tumor ADC histogram-derived parameters, such as the volume, ADC mean , the 10th, 50th and 90th percentile ADCs, inhomogeneity, skewness, kurtosis and entropy, were compared among SMBT, CCC and EC. The diagnostic e cacy of these parameters was evaluated using receiver operating characteristic curve analysis.Results: There were signi cant differences in age, ADC SC and histogram parameters including volume, ADC mean , the 10th, 50th and 90th percentile ADCs among three kinds of tumors (all p < 0.05). Patients with SMBT were signi cantly younger than patients with CCC/EC. The signi cantly higher ADC SC and smaller volume were found in SMBT than in CCC/ EC. The ADC mean was signi cantly higher in CCC than in EC. The 10th percentile ADC was signi cantly lower in EC than in SMBT/CCC. The 50th and 90th percentile ADCs were signi cantly higher in CCC than in SMBT/EC. For differentiating SMBT from CCC, AUCs of the ADC SC , volume, the 50th and 90th percentile ADCs were 0.97, 0.86, 0.72 and 0.81, respectively. For differentiating SMBT from EC, AUCs of the ADC SC , volume and the 10th percentile ADC were 0.97, 0.71 and 0.72, respectively. For differentiating CCC from EC, AUCs of the ADC mean , the 10th, 50th and 90th percentile ADCs were 0.79, 0.72, 0.81 and 0.85, respectively.Conclusions: Similar with ADC SC , whole-tumor ADC histogram analysis was valuable for differentiating endometriosis-related tumors. It was helpful for the 50th, 90th percentile ADCs and volume in differentiating SMBT from CCC; for the volume and 10th percentile ADC in differentiating SMBT from EC, and for the ADC mean , the 10th, 50th and 90th percentile ADCs in differentiating CCC from EC.
