The human IgG and IgA response was determined after vaccination with an experimental 24-valent Klebsiella capsular polysaccharide vaccine. The majority of natural antibody acquired prior to immunization was found in the IgG1, IgG2 and IgA1 subclasses. The immune response to vaccination was concentrated within the IgG1, IgG2, IgA1 and IgA2 subclasses with greater than or equal to 70% of subjects responding with a significant (greater than or equal to fourfold) rise in titer. The IgG3 and IgG4 response was meager with few volunteers (0%-40%) showing a significant titer rise. Therefore, vaccination with Klebsiella capsular polysaccharide induces a vigorous IgG and IgA response in humans which is not restricted to single antibody class or subclass.

Intravenous immune globulin (IV-IgG) is approved for use in substitution therapy of immunodeficiency syndromes and treatment of idiopathic thrombocytopenic purpura (ITP). The initial dose for substitution therapy is 0.2 g/kg body weight repeated monthly. If necessary, the dose may be increased to 0.3 g/kg and the frequency to every 2-3 weeks. The approved dosage for ITP is 0.4 g/kg daily for 2 to 5 consecutive days. Repeat doses of 0.4 g/kg have been used as maintenance therapy. The initial infusions of IgG to immunodeficient patients who have not been treated during the previous 8 weeks or never treated at all should proceed with caution. These patients are at risk of potentially serious complement-mediated adverse reactions. Adverse reactions due to IgG administration are less frequent and less severe for patients with ITP. Currently available preparations are contraindicated in patients with a selective IgA deficiency and detectable IgA antibodies.

A human monoclonal antibody to cytomegalovirus (CMV) was produced by transforming peripheral blood mononuclear cells of a patient with recent CMV infection. It is directed against a late antigen located in the nucleus of CMV infected fibroblasts at 24-72 hours postinfection and immuneprecipitates 65K and 48K proteins from 35S-labelled CMV infected cells. Results of its use in an early nuclear fluorescence assay for rapid diagnosis are presented.
