Barotrauma after liquid nitrogen ingestion: a case report and literature review

Abstract: A 25-year-old man developed a gastric perforation after ingesting a homemade drink containing liquid nitrogen. Surgical repair had to be postponed to seek consultations with experts because the available practitioners in this case, including emergency physicians, surgeons, and anesthesiologists, had little experience and knowledge about the source of the patient's pneumothorax and subcutaneous emphysema. The patient ultimately underwent exploratory laparotomy with general anesthesia, considering that delaying … Show more

“…Although individuals may also be exposed to LN 2 in nonretail food settings (e.g., schools, homes, etc. ), these types of encounters are very rare (23,43). Additionally, LN 2 is not readily available for purchase at most stores because very few specialty retailers and distributors sell the agent.…”

“…L iquid nitrogen (LN) is a colorless, odorless, nontoxic, cryogenic fluid that rapidly evaporates when exposed to room temperature, resulting in a smoke-like effect due to its extremely low boiling point of −196°C. [1][2][3][4][5][6][7][8] This is exploited by establishments that prepare frozen desserts, cocktails, and snacks. One popular snack marketed in the United States as "Dragon's Breath," "Heaven's Breath," or "Nitro Puff " attracts young patrons in malls, food courts, kiosks, fairs, and food retail locations.…”

“…Liquid nitrogen ingestion can result in mucosal thermal injury and barotrauma as the liquid to gas expansion ratio results in volume increase at a rate of 1 to 694. [1][2][3][4][5]8 Hollow viscous injury associated with LN ingestion affects the stomach, causing it to distend and rupture generally along the lesser curvature where it is fixed by the left gastric artery, celiac trunk, and gastroesophageal junction. [1][2][3][4] Prompt medical evaluation and surgical consultation are paramount after ingestion of LN in symptomatic patients.…”
