Neuropsychiatric Symptoms and Caregiver Burden in Individuals With Alzheimer’s Disease: The TEAM-AD VA Cooperative Study

Abstract: Neuropsychiatric symptoms were prevalent in both mild and moderate AD, even in patients receiving treatment with an acetylcholinesterase inhibitors, and were more severe in moderate AD and associated with greater caregiver time.

“…There is a substantial body of research related to caregiver burden, ISBs, and NPI scores in the AD group. 4,[18][19][20]22,23,36,37 The prevalence of ISBs, and their relationship with NPSs, were investigated in most of the studies examining the former. 18,19,26,38 The prevalence of ISBs among patients with dementia was reported to range between 7% and 25%.…”

“…3 Behavioral and psychiatric symptoms, which are collectively referred as neuropsychiatric symptoms (NPSs), are among the most common manifestations of AD. 4 Neuropsychiatric symptoms are seen more frequently in patients with AD than in the general population. Whereas the prevalence of NPSs has been reported as 16% to 27% in community-based studies, the prevalence among people living with AD is between 52% and 77%.…”

“…Prevalence rates vary depending upon the instrument used for assessment but some recent estimates suggest apathy might occur in as many as 49% of patients with Alzheimer's Disease (AD) ( Zhao et al., 2016 ), 40% with Parkinson's Disease (PD) ( den Brok et al., 2015 ), and 38% of individuals with late-life depression ( Yuen et al., 2015 ). The syndrome is associated with greater cognitive decline, increasing disease severity, poorer quality of life and more caregiver burden in AD and PD ( Chen et al., 2018 ; Martinez-martin et al., 2015 ). It is therefore emerging as an important factor to stratify severity and prognosis in many different patient groups.…”

“…It has been postulated that caregiver burden mediates the effect of certain risk factors on institutionalization, particularly on the issues of neuropsychiatric symptoms (de Vugt et al, 2005; Okura et al, 2011; Springate & Tremont, 2014; Terum et al, 2020). Neuropsychiatric symptoms are associated with greater caregiving time and cause a higher caregiver burden (Chen et al, 2018). Therefore, the aims of the study were to (1) probe the institutionalization rate in patients with dementia in Taiwan, (2) identify the predictors of institutionalization, and (3) conduct a mediation analysis of caregiver burden between neuropsychiatric symptoms and institutionalization.…”
