Clinical characteristics and bisphosphonates treatment of rare pregnancy- and lactation-associated osteoporosis

Abstract: Pregnancy- and lactation-associated osteoporosis (PLO) is a rare disorder with poorly known etiology, pathophysiology, and therapy. We aimed to investigate the clinical characteristics of PLO and evaluate the effectiveness and safety of bisphosphonates on it. A total of 12 patients were diagnosed with PLO on the basis of medical history, bone mineral density (BMD), and/or fragility fractures during pregnancy and lactation. We investigated the clinical, biochemical, and radiological characteristics of patients.… Show more

“…Stopping breastfeeding could reduce the loss of calcium, and the longer of the lactation lasting, the more fractures occur on average [13]. Bisphosphonates have a relatively short bone retention time and are highly effective and well tolerated in the treatment of PLO, but they need to be cautious or stop breastfeeding because of their potential harm to the fetus [14,15]. Most of the patients could not return to the normal level of BMD after treatment, and the BMD does not decrease again during the second pregnancy, but the safety of re-pregnancy needs more prospective clinical study.…”

“…Do tej pory opisano ponad 300 przypadków tej choroby. Najbardziej liczne opisy serii przypadków dotyczą 107 [4], 52 [1], 34 [5], 32 [6], 24 [2], 13 [7] i 12 [8] kobiet. Dolegliwości są następstwem niskoenergetycznych złamań trzonów kręgowych, których przyczyną jest zmniejszenie mineralnej masy kości i obciążenie mechaniczne w ciąży.…”

“…Złamania szyjki kości udowej, żeber, kości krzyżowej, kości stóp i innych, opisywano u pojedynczych osób [1,4,7]. Badania BMD za pomocą DXA, wskazują na znaczny niedobór masy mineralnej, zwłaszcza w częściach szkieletu z dużą zawartością kości beleczkowej, czyli w trzonach kręgowych i w obrębie szyjki kości udowej [1,2,7,8]. Niewiele doniesień podaje wartości BMD dla kości korowej przedramienia, które są prawidłowe [2].…”
