Purpose: To compare the effect of photodynamic therapy (PDT) as a procedure to complement root canal treatment (RCT), for both primary treatment and retreatment. Methods: This was a retrospective study based on analysis of clinical records. A total of 214 teeth that had undergone RCT on either a primary or retreatment basis, with or without complementary PDT, were evaluated. For 118 teeth that met the previously established inclusion criteria, the time until healing was evaluated. Complementarily, the need for application of calcium hydroxide (CaHy) between visits and the number of visits necessary for completing the treatment were assessed. Data were analyzed using the Mann-Whitney and χ 2 tests with a significance level of P < 0.05. Results: Periapical radiolucency resolution was achieved at 15 ± 9.33 months in the RCT + PDT group and 20.35 ± 22.1 months in the RCT group (P = 0.07). For primary treatment, CaHy was necessary in 72.4% of the RCT cases and 16.4% of the RCT + PDT cases (P < 0.01). For retreatment cases, CaHy was used in 82.7% of the RCT cases and 17% of the RCT + PDT cases (P < 0.01). In the RCT group, more than two visits were necessary for primary treatment in 18.6% of the cases, compared with 13.10% in the RCT + PDT group (P = 0.31), whereas for retreatment, more than two visits were necessary for 64.9% and 49.1% of cases, respectively (P = 0.05). Conclusion:In comparison with RCT alone, teeth receiving RCT + PDT showed less variation in the time needed for periapical lesion healing, fewer cases required CaHy, and fewer cases required more than two visits to complete the treatment.

Uma revisão sistemática foi realizada para avaliar a redução de microrganismos intracanal em dentes com necrose pulpar com o uso da terapia fotodinâmica (TFD) comparada ao hidróxido de cálcio (HC). A pesquisa buscou ensaios clínicos randomizados que empregaram a TFD para auxiliar na desinfecção intracanal em dentes com necrose pulpar comparada com o uso do HC. A busca foi realizada no PubMed, Scopus, Web of Science, LILACS, BBO, Cochrane Library e literatura cinzenta. Resumos do IADR, registros de ensaios inéditos, dissertações e teses também foram pesquisados. A ferramenta de análise de risco de viés da Cochrane Collaboration foi aplicada para a avaliação da qualidade dos estudos e a qualidade de evidência com o GRADE. Foram identificados 121 artigos. Apenas 5 estudos permaneceram na síntese qualitativa, sendo que um foi considerado de “baixo” risco de viés e 4 considerados com risco de viés “indefinido” nos domínios-chaves. Os 5 artigos foram incluídos na meta-análise. A redução de microrganismos em dentes com necrose pulpar comparando a TFD em relação ao HC foi de 0,01 (intervalo de confiança [CI] = -0,04 a 0,07, p = 0,59). A qualidade de evidência foi classificada como moderado para os tratamentos. Esse resultado demonstra que não houve diferença entre as terapias. Pode-se concluir que não houve diferença entre o uso da TFD ou do HC na desinfecção intracanal de dentes com necrose pulpar.

This article presents results obtained from a survey, including patients who underwent endodontic treatment by the single-visit or multi-visit method, after confirmation of the diagnosis of chronic apical periodontitis.Objective: The aim of the survey was to obtain data from the studied patients on the frequency and the type of postoperative pain after treatment of chronic apical periodontitis, as well as whether there is a relation between gender, age, and postoperative pain.Materials and methods: A visual analog scale was used to study the intensity of postoperative pain in the treatment of teeth diagnosed with CPP, which are treated by one of two methods -single-visit or multi-visit method. The total number of surveyed patients is 71. The patients were examined and treated at the Dental Clinic "Imperial" in Varna, Bulgaria, in 2020. Thirty-one of them were treated by the single-visit method, and the remaining 40 by the multi-visit method with placement of a temporary dressing or sterile swab.Results: A relatively large proportion (70%) of patients reported mild pain immediately after the root canal filling. A relatively large proportion (90.3%) of patients did not report pain one week after the root canal filling. The more frequent symptoms were observed in cases treated by the multi-visit method, after the application of a temporary dressing. Patients who reported taking analgesics were treated in the multivisit method. More frequent pain symptoms with both methods of treatment were observed in men aged 36-60 years. Conclusion:Although exacerbation has been shown to have no significant effect on the outcome of endodontic treatment, it is highly undesirable. In the short term, the postoperative pain in patients treated by the multi-visit method through the use of intracanal medication is more pronounced. Patients receiving the single-visit treatment reported less postoperative pain.
