The Anesthetic Management for a Patient With Trisomy 13

Abstract: Trisomy 13 is a chromosomal disorder that occurs in complete or partial mosaic forms. It is characterized by central apnea, mental retardation, seizure and congenital heart disease. The survival of the patients with trisomy 13 is the majority dying before one month. Trisomy 13 is the worst life prognosis among all trisomy syndromes. It is reported the cause of death is central apnea. Special needs patients with mental retardation are recognized to have poorer oral health condition. Oral health related quality … Show more

“…It is possible that the hearing aid contributed to the better intelligence development of patient 7 compared to patient 8. Summarizing the reports describing the development of patients with trisomy 13 over 5 years old (Table 5), 7,[11][12][13][14][15] only one patient was able to sit with support, despite the presence of holoprosencephaly. In addition, patients who smiled and made visual contact were those with no or mild brain anomalies.…”

“…Thus far, only a few reports have described the activities of daily living (ADL), intellectual ability and/or management of home care of long-term survivors with trisomy 13. 7,[10][11][12][13][14][15] Furthermore, there are no reports on psychomotor developmental progress of such patients. To plan effective treatment and management of patients with trisomy 13, it is necessary to clarify not only the prognosis for life, but also the patients' living situations after discharge and their developmental status.…”

“…We did not provide her premedication before the induction of general anesthesia. General anesthesia was induced with sevoflurane up to 5% and 33% nitrous oxide in oxygen [11]. OSA made mask ventilation difficult even though two anesthesiologists were involved in positioning the head, moving the mandible forward, and opening the mouth to facilitate ventilation.…”

“…Highdose corticosteroid therapy has been reported to be helpful. In anesthetic management, it is essential to perform routine monitoring of the peripheral capillary oxygen saturation (SpO 2 ) and cardiac function (CO [cardiac output], CI [cardiac index], SV [stroke volume], and SVV [stroke volume variation]), using Aesculon ® a noninvasive cardiac function monitor that uses the electrical velocimetry method to measure aortic blood flow, or the NICOM ® which measures the bioreactance, or the phase shift in voltage, across the thorax [35,36]. Hence, we should be aware of the possible development of acute respiratory failure after procedures especially in emergency cases.…”
