Objetivo: Explorar la relación entre los sistemas de activación de conducta (SAC) / sistema de inhibición de conducta (SIC) y funciones ejecutivas (FE) en excombatientes del conflicto armado colombiano. Metodología: Estudio observacional, exploratorio, con muestra no aleatoria de 39 excombatientes del programa de la Alta Consejería de la Presidencia para la Reintegración, residentes en Antioquia-Colombia, edad 35±7.6 años, 87.2% hombres, y escolaridad 10±2.8 grados; comparados con un grupo control de 14 no combatientes (edad 33±6.1 años, 71.4% hombres y escolaridad 10±2.7 grados). Se les administró un cuestionario de SAC/SIC, usando la sensibilidad a la recompensa (SR) y la sensibilidad al castigo (SC). Para la FE se aplicó la prueba Ineco Frontal Screening (IFS). Resultados: Hubo alteraciones específicas de la FE: en la ejecución de instrucciones conflictivas (IC), memoria de trabajo verbal (MTV) y control inhibitorio verbal (CIV). Se evidenciaron correlaciones directas, con coeficientes de moderados a altos entre el SAC (SR) y la capacidad de abstracción CA, el control motor inhibitorio (CMI), la MTV, la memoria de trabajo espacial (MTE) y el funcionamiento ejecutivo global (IFSTOT). Conclusiones: Los excombatientes presentaron un rendimiento inferior en la memoria de trabajo verbal y el control inhibitorio verbal, lo que impacta las capacidades de anticipación, establecimiento de metas, diseño de planes, iniciación de conductas voluntarias y el control inhibitorio. Lo anterior puede generar la toma de decisiones equivocadas.

IntroductionDecisions under ambiguity occurs daily for everyone. Subsequently, we all deliberate upon options to initiate an action most appropriate for current goal demands. Researchers has attempted to identify factors which contribute to risk taking, alongside the neurocircuitry underpinning it. Empirically, uncertain decision making is frequently assessed using the Iowa Gambling Task (IGT). Research have reliably identified varying regions implicating two broader circuits known as the reward and salience networks. However, considerable work has focused on contrasting “good” versus “bad” decisions.MethodsThe present investigation attempted a unique approach to analyzing the modified IGT acquired during fMRI (n = 24) and focused on active and passive bad decisions to identify potential internetwork connectivity, dissociable connectivity patterns between approach and avoidant bad decisions, and their relationship with personality traits, which can be linked with behavioral approach styles.ResultsNetwork cluster analyses revealed general internetwork connectivity when passing (avoiding) good decks; however, the OFC was functionally disconnected from the rest of the selected brain regions when playing (approaching) bad decks. Decreased reward responsiveness was linked to increased functional connectivity between the lateral OFC and aSMG, while drive was associated with increased functional connectivity between dACC and aINS.DiscussionWe report evidence that approach and avoidant bad decisions are associated with distinct neural communication patterns. Avoidant decisions were marked by substantial network integration and coherence, contrasted with the general scarcity of internetwork communication observed for approach decisions. Furthermore, the present investigation observed preliminary evidence of personality traits linked with neural communication between salience and reward evaluative networks.

A number of studies of posttraumatic stress disorder (PTSD) report thinner cerebral cortical gyri using gyrus-based analysis or thinner foci within the gyri using vertex-based analysis. However, the locations of these findings are inconsistent across studies, and the spatial transformations required during vertex-based analysis may affect the focal findings. A mega-analysis using a large number of subjects from multiple PTSD studies could potentially identify more reproducible cortical thickness abnormalities. Investigating both the vertex and gyral thicknesses simultaneously may verify the vertex-based focal findings using gyral data without imposing any spatial transformation. Here we aggregated data from 24 international laboratories using ENIGMA standardized procedures for 949 adult PTSD patients and 1493 controls without PTSD (age 18 to 65 years). We examined whether gyral and vertex cortical thickness are (a) different between subjects with PTSD and controls and (b) associated with PTSD symptom severity in trauma-exposed subjects. Regions with overlapping thinner cortical gyri and thinner vertex clusters were located in frontal, temporal, parietal, and occipital cortices. Thinner right lateral orbitofrontal and right lingual gyri and concomitantly thinner vertex clusters in the anterior portions of both gyri were associated with PTSD symptom severity. Convergent findings in these locations suggest focally thinner cortex in these gyri, which may be involved in altered processing and regulation of emotion and sensory inputs underlying posttraumatic stress symptoms.
