Distant metastases in head and neck cancer

Abstract: Distant metastases in head and neck cancer led to dismal prognosis. Factors associated with distant metastasis are related to characteristics of the primary tumor. © 2017 Wiley Periodicals, Inc. Head Neck 39: 1733-1743, 2017.

“…In total, 20 (19%) patients developed distant metastasis, while only half of these patients developed a loco-regional recurrence, indicating early development of occult metastasis. The prevalence of distant metastasis seems somewhat higher in our series than in other studies, being 8% to 13.7% [27][28][29]. Duprez et al reported a 2and 5-year actuarial rates of distant control of 84% and 80% in a cohort of 1022 patients with HNSCC [29].…”

“…Primary tumors that metastasize to the thyroid are often found in the lungs or kidneys [12]. Since distant metastases of head and neck cancers are commonly found in the lungs, bone, liver, and skin, in decreasing order of frequency, metastasis to the thyroid gland is extremely rare [13]. Among the head and neck cancers, nasopharyngeal carcinoma is the most commonly reported primary lesion that metastasizes to the thyroid gland [4,14,15].…”

“…OSCC metastasis to the thyroid gland is a distant metastasis, and the current case was stage IVb T4aN2cM1, according to the tumor-node-metastasis classification. The prognosis of HNSCC with distant metastasis is generally poor [13]; however, compared with HPV-negative OSCC, control of the distant metastatic lesions is reportedly better in HPV-positive OSCC cases [13,18]. Nonetheless, whether the prognosis of HPV-positive OSCC with thyroid gland metastasis is good remains unknown.…”

“…Clinical or pathologic stage is valuable for predicting the prognosis of patients with OSCC. However, the survival outcomes can be quite different even for patients with same stage cancer . We have tried to uncover the role of serum markers in the prediction of survival in patients with OSCC.…”

“…However, the survival outcomes can be quite different even for patients with same stage cancer. 17,18 We have tried to uncover the role of serum markers in the prediction of survival in patients with OSCC. Previously, we found that higher serum lipid Lp (a) was associated with poorer overall survival.…”
