Objetivo: conocer la prevalencia del síndrome genitourinario de la menopausia en una población de mujeres sexualmente activas, con edad igual o mayor a 40 años.Materiales y métodos: estudio de corte transversal descriptivo. Se incluyeron mujeres diagnosticadas con menopausia, con actividad sexual en las últimas seis semanas, y residentes en el Quindío, departamento ubicado en la zona andina cafetera en Colombia. Se excluyeron mujeres con menopausia quirúrgica, analfabetas, con padecimientos de trastornos mentales o déficit mental. El estudio se realizó en una institución privada de referencia entre el 2013 y 2016. Se hizo un muestreo consecutivo por conveniencia. Se midieron variables sociodemográficas, síntomas vulvo-vaginales y síntomas del tracto urinario inferior relacionados con la presencia del síndrome genitourinario de la menopausia, presencia de disfunción sexual global por medio del Índice de Función Sexual Femenina validado en español, y por tipo de disfunción. Se realizó un análisis descriptivo de la información utilizando medidas de frecuencia absoluta y relativa. Se calculó la prevalencia como: número de mujeres con síndrome genitourinario de la menopausia / número de mujeres encuestadas.Resultados: de un total de 597 mujeres seleccionadas para participar, se analizaron finalmente 558 (93,46 %). La edad promedio fue de 59,73 ± 8,4 años. La prevalencia del síndrome genitourinario de la menopausia fue del 51,61 % (62,88 % en las mayores de 55 años y 39,32 % en las menores de 55 años). La prevalencia de disfunción sexual en el grupo estudiado fue del 67,2 %. Se presentaron trastornos en el deseo (67,2 %), la excitación (45,16 %), el orgasmo (44,08 %), la lubricación (32,25 %) y dolor durante la relación (13,97 %).Conclusión: en el grupo de población evaluado se encontró una importante prevalencia del síndrome genitourinario de la menopausia y de disfunción sexual. Se requieren estudios poblacionales para hacer una aproximación a la magnitud del fenómeno en la región latinoamericana.

GSM includes wide spectrum of vulvovaginal Symptoms and urinary troubles replacing the term vulvovaginal atrophy (VVA). It is a silent epidemic condition affecting 50-60% postmenopausal women. Estrogen withdrawal causes thinning, narrowing, tissue loss & reduced blood supply in vulvo-vaginal area, which results GSM. GSM causes burning in vagina, dyspareunia, urinary urgency, repeated UTI. Dyspareunia affects all the domain of sexual function and deteriorates the quality of life. Irony is, women are oblivious to share and doctors are reluctant to discuss. So women keep continue suffering without knowing the restorative treatment. Repercussion of GSM/VVA intensifies the sorrows, distress and sufferings. It has profound effect on relationship and psychology and quality of life of women. Good history taking and clinical examination do diagnosis. Investigations are done to exclude other causes. Treatment is challenging. Maintenance of optimum body weight, exercise, regular coitus, quitting smoking & excessive alcohol intake are the key factors. Vaginal moisturizers are recommended as 1st line therapy for mild to moderate VVA or women who can't take estrogen. Ideal moisturizers should have similarity with vaginal secretion of osmolality, pH and composition. Meta analysis shows local estrogen therapy is effective. It restores vaginal pH and maturation index. Systemic absorption is minimal so progesterone needs not to be added. Testosterone improves dyspareunia, sexual desire, lubrication and satisfaction. DHEA (Prasterone) penetrates vaginal wall better. It increases elasticity and vascularity of vagina. RCTs have not shown benefits of it's systemic therapy. But local daily administration of DHEA reduces dyspareunia and GSM so improves the quality of life. Ospemifene is well tolerated. It's agonist effect on vaginal mucosa and antagonist effect on endometrium and breast, makes it promising. Lasofoxifene, third generation SERM, is also very effective but it needs FDA approval. Laser is widely being used and very effective. It is simple, faster, painless procedure. It activates dominant fibroblasts, proteoglycans, hyaluronic acid, thereby improves GSM & sexuality. Black cohosh, Botox, G-shot, probiotics, gabapentin are not yet evidence based. Still there is significant unmet need for medical treatment. Women reports GSM but that is only tip of iceberg. Good communication and optimum treatment only can break the sorrows GSM/VVA.

Vulvovaginal atrophy accompanied by lower urinary tract dysfunction related to low levels of estrogen and androgens is labeled as genitourinary syndrome of menopause (GSM). Although this condition affects most postmenopausal women worldwide, it seems to be underdiagnosed and undertreated. Women should be properly advised to choose an adequate treatment modality to improve their quality of life, sexual relationships and social activity. The aim of this article to is increase knowledge of GSM. The current treatment options, both hormonal and non-hormonal, are reviewed. Topical estrogen therapy still remains the gold standard, but the demand for individually tailored therapy is growing. New treatment modalities are continuously included in clinical practice. They should consider the whole personality of a woman as well as cultural and social factors. Further studies on GSM and on the effectiveness of various treatment options are necessary to achieve this purpose.

Abstract: GSM is underdiagnosed and undertreated in Asia. As discussion of GSM with HCPs appears to be a factor influencing women's awareness and treatment status, a more active role by HCPs to facilitate early discussions on GSM and its treatment options is needed.
