Sleep Disordered Breathing in Duchenne Muscular Dystrophy

Abstract: This review aims to explain the inevitable imbalance between respiratory load, drive, and muscular force that occurs in the natural aging of Duchenne muscular dystrophy and that predisposes these patients to sleep disordered breathing (SDB). In DMD, SDB is characterized by oxygen desaturation, apneas, hypercapnia, and hypoventilation during sleep and ultimately develops into respiratory failure during wakefulness. It can be present in all age groups. Young patients risk obstructive apneas because of weight gai… Show more

“…Irreversible muscle degeneration and replacement by adipose and connective tissue occur with advancing age in DMD patients [37]. Obesity is common in DMD boys because of the forced continuous physical inactivity and long-term use of corticosteroid therapy [38]. These processes explain the excess of adipose tissue in the patients studied, which is related to the progression of the disease and the lifestyle associated.…”

“…Dystrophin deficiency in DMD results in mechanical deficits in striated muscle [5]. Respiratory muscle weakness occurs in DMD, with major implications for respiratory control [37]. Inspiratory muscle strength declines with increasing age in DMD boys [6].…”

“…Inspiratory muscle strength declines with increasing age in DMD boys [6]. Respiratory disturbances are common during sleep, including hypoventilation [38,39] and apnoeic events [37,40,41]. The mdx mouse model of DMD has characteristics similar to DMD, including disruptions to normal breathing [12,[42][43][44] and diaphragm and pharyngeal dilator muscle weakness [12,15,16,32,45].…”
