Changes in bone turnover markers with HIV seroconversion and ART initiation

Abstract: HIV seroconversion was associated with decreased OC levels while ART initiation was associated with decreases in sclerostin, a negative regulator of bone formation. Our results suggest that both HIV infection and ART have an impact on bone metabolism in white men.

“…While literature findings on adult patients provide contrasting results, showing both increase and decrease of bone formation markers [16, 17], studies on pediatric populations show an increase in both formation and resorption markers. In a study comparing 32 patients with vertically transmitted HIV, in ART (age 6.3-17.7 years), with healthy controls, Mora and colleagues reported higher values of BALP (formation marker) and NTX (resorption marker) in the HIV positive group [18].…”

“…All articles in this category found significantly reduced BMD, altered bone metabolism, or higher rates of [30][31][32][33][34][35]. Four articles rated as moderate to high quality [30,[33][34][35]. Of note two further articles rated low to moderate quality (AXIS score 5-10), rendering them the two lowest rated articles in this review [31,32].…”

“…HIV-infected patients are believed to be three times more likely to present osteoporosis than non-HIV-infected patients [23]. HIV infection and the use of antiretroviral drugs (ART) modifies the regulatory markers of bone absorption and resorption, leading to lower BMD [25].…”
