Patients with Small Abdominal Aortic Aneurysm are at Significant Risk of Cardiovascular Events and this Risk is not Addressed Sufficiently

Abstract: Previous data suggest that patients with small AAA have a high risk of cardiovascular (CV) mortality and morbidity. The recent implementation of the NHS AAA screening programme (NAAASP) and similar programmes elsewhere has led to several individuals being diagnosed with small AAA, yet addressing their CV risk-factors is still not formalised clinical practice within screening programmes. The precise contemporary CV risk-profiles of these patients also remain unknown. Our findings suggest that despite recent adv… Show more

“…In the NHS AAA Screening Programme (NAAASP), there are currently almost 13,000 of these men who are offered regular surveillance and advice from a vascular nurse. Rupture rates are reassuringly low (<0.5%) in these men (unpublished data), but they are at risk of cardiovascular complications, 9 and their optimal management is unknown. It is suggested that men in surveillance who are taking antiplatelet and statin therapy have a lower risk of death from cardiovascular complications.…”

“…Repairing small aneurysms (under 5.5 cm) has no known benefit [18]. However, early detection provides an opportunity for close monitoring and intervention (eg, smoking cessation) to reduce accelerated aneurysm growth and overall cardiovascular risk [19,20]. Nevertheless, guidelines on optimal AAA follow-up remain inconclusive, and follow-up rates are poor, often owing to referring provider failure to order repeat scans [21,22].…”
