Background: Various methods of laparoscopic appendectomy have been described in children. We present the data of 50 children who underwent interval appendectomy at our institution by transumbilical single-incision laparoscopy-assisted appendectomy (SILAA). Materials and methods: Fifty patients <12 years from June 2011 to June 2017 with inclusion criteria <12 years of age who were admitted with clinical features of acute appendicitis of >24-48 hours' duration; had abdominal ultrasound (USG) with appendicular diameter of >10 mm and good clinical response to initial management by intravenous antibiotics within 24-48 hours of admission were retrospectively analyzed. They underwent SILAA after 6 weeks. Under general anesthesia, an infraumbilical incision was made and umbilical tube was identified. A 5 mm camera port was inserted by open Hassan's technique. After visualizing the appendix, another incision was made adjacent to the port site on the left and a 5 mm instrument was introduced through this. The appendix was freed, mobilized, and delivered through the incision. Appendectomy was completed extracorporeally. Results: The average age at presentation was 9.3 years. There were 18 females and 32 males. Two patients required conversion to open procedure in view of extensive adhesions and a short retrocecal appendix which was difficult to mobilize and exteriorize through umbilicus. The mean operating time was 30 minutes. There were no complications. Conclusion: Single-incision laparoscopy-assisted appendectomy combines the advantages of both laparoscopic and open appendectomy and offers reduced operative time and less complications and reduced surgical costs in pediatric age group.

Background: Appendicitis is one of the common pathologies encountered in surgical practice. Except in minority of the cases, the treatment is usually surgical. Till recent past, open appendectomy has been the procedure of choice for appendicitis. With increasing expertise in laparoscopic surgeries more and more surgeons are utilizing the laparoscopic approach for appendectomy. Laparoscopic surgeries have distinct advantages such as less surgical trauma, improved and quick postoperative recovery, and esthetic results. Single-incision laparoscopic surgery (SILS) is rapidly gaining acceptability in young population because of its cosmetic advantages. Moreover, these surgeries also avoid the risk of port-site hernias and the possibility of wound infection. Aims and Objective: The purpose of this study is to present our initial experience with this surgery using a single incision laparoscopic appendectomy (LA) using conventional instruments. Materials and Methods: This was a prospective cohort study conducted in the department of surgery of a tertiary care medical college situated in an urban area. The duration of the study was 2 years. All adult patients diagnosed to be having uncomplicated appendicitis and undergoing appendectomy by SILS were included in this study on the basis of a predefined inclusion and exclusion criteria. Pre-operative data collected included age, sex, weight, duration of complaint, concomitant medical conditions (like ischemic heart disease, chronic obstructive airway disease, diabetes mellitus, pancreatitis, and liver cirrhosis) and previous upper or lower abdominal surgery. All patients were treated by SILS except 1 patient in whom the procedure was converted to open surgery. Mean surgical time, Intraoperative procedure details and postoperative complications were studied in all the cases. p < 0.05 was taken as statistically significant. Statistical analysis was done using SSPS 21.0 software. Results: Out of 30 patients in this study 26 patients were female and 4 patients were male. The male to female ratio was found to be 1:6.15. Mean age of studied cases was found to be 26.2 years. Operative time required for the first 15 cases in an average was 120.00 min however it was reduced for the next 15 cases was 73.73 min. Overall time required in an average was 96.86 min. Out of 30 cases, The procedure was completed with Single Incision LA in 23 Patients, i.e., 76.6 %. In the initial cases, we started with two 5 mm and one 10mm port. To reduce crowding we replaced the 5 mm port to 3 mm port. The 10 mm port was also replaced by 5 mm in the past few cases in 1 patient the procedure was converted to open surgery. The analysis of postoperative complications showed that five patients had Post-Operative wound Infection. One patient had post-operative peritonitis. Conclusion: SILS is a feasible and safe surgical method for appendectomy and is being increasingly preferred particularly by young patients due to its excellent cosmetic results.

Мета роботи: оцінка ефективності використання однопортового лапароскопічного доступу в пацієнтів, хворих на гострий апендицит. Матеріали і методи. Проведено порівняльний аналіз показників 74 хворих оперованих із приводу гострого апендициту впродовж 2020–2022 рр. за критеріями: характеристика пацієнтів (вік, стать, ІМТ), тривалість операційного втручання та перебування в стаціонарі, дренування черевної порожнини, анестезіологічна оцінка ASA, рівень больового синдрому та використання опіоїдних анальгетиків у післяопераційний період, наявність інтра- та післяопераційних ускладнень, косметичний результат. З включенням пацієнтів з апендицитом без місцевих ускладнень та терміном захворювання до 48 год; виключенні хворі з перфорацією червоподібного відростка, періапендикулярним абсцесом, поширеним перитонітом. У 33 (44,6 %) пацієнтів (1-ша група) виконано однопортову лапароскопічну апендектомію (SILА), у 41 (55,4 %) хворих (2-га група) – багатопортова лапароскопічна апендектомія (MLA). Результати досліджень та їх обговорення. Найбільш значні відмінності результатів лікування виявлені у першої групи пацієнтів (SILA) стосовно рівня больового синдрому та задоволеності косметичним результатом. При виборі методу операційного втручання необхідно провести ретельну оцінку пацієнта. Важливе місце займають вік хворого, стать, ІМТ та наявність місцевих ускладнень (апендикулярний інфільтрат, абсцес, злукова хвороба). Термін перебігу хвороби понад 48 год та ускладнення гострого апендициту визначають необхідність виконання багатопортової лапароскопічної апендектомії. У разі наявності пупкової грижі у хворого слід використовувати однопортовий доступ, що дає можливість досягти герметичності в грижових воротах для підтримання достатнього пневмоперитонеума. Виконання конверсій однопортового лапароскопічного доступу здебільшого зумовлено технічними складнощами через неможливість досягнення адекватної експозиції апендиксу та труднощами при його мобілізації. В таких випадках можна встановити додатковий троакар або перейти на багатопортовий лапароскопічний доступ, іноді лапаротомію.
