PurposeHyperleptinemia has been independently associated with human cardiovascular (CV) diseases. Accordingly, we evaluate the association between serum leptin and future CV events in patients with coronary artery disease (CAD).Patients and methodsThis study enrolled 98 patients with CAD from January to December 2012. The primary endpoint included incidences of major adverse CV events and hospitalization. Patients follow-up had been completed on June 30, 2017.ResultsAfter a median follow-up of 52 months, 43 CV events had occurred. Patients with CV events had higher systolic blood pressure (P = 0.030), total cholesterol (P = 0.034), C-reactive protein (P = 0.018), and serum leptin levels (P = 0.001) than those without CV events. Kaplan–Meier analysis showed greater cumulative incidences of CV events in the high leptin group (median leptin concentration >6.03 ng/mL) than in the low leptin group (log-rank P = 0.012). Multivariate Cox regression analysis showed that triglyceride (HR: 1.010; 95% CI: 1.001–1.018; P = 0.022) and leptin levels (HR: 1.054; 95% CI: 1.026–1.082; P < 0.001) were independently associated with CV events in patients with CAD.ConclusionSerum leptin levels could serve as a biomarker for future CV events in patients with CAD.

Aim.To consider an opportunity for application of different combinations of biochemical and bioimaging parameters to create different multimarker diagnostic panels designed for assessment of risk of coronary atherosclerosis and its complications.Material and methods.To the analysis, data included, obtained from patients 18 y.o. and older (n=502), investigated at NMRCPR of the Ministry of Health in 2011-2013, who had undergone diagnostic coronary arteriography and duplex carotid scanning. Atherosclerosis burden was measured according the Gensini score. Subfractional spectrum of apoB-lipoproteides was assessed with the Quantimetrix Lipoprint LDL System (USA), biochemistry was done with standard lab. methods. Statistics was done with software Statistica v.10, IBM SPSS Statistics v.20, SAS v.9.4.Results.Several multimarker combinations (panels) for non-invasive estimation of risk of coronary atherosclerosis detection and its severity were proposed. These are 1) an index K, calculated as ratio of the sum of potentially atherogenic subfractions to the large physiologically active LDL1 particles; index K >1,7 indicates an increased atherogenic potential of apo B-containing particles even with normal lipid profile, and can be used for non-invasive prediction of coronary atherosclerosis; 2) duplex complexes as adiponectin to endothelin ratio which <7,0 is associated with coronary atherosclerosis risk only in men, and as leptin to insulin ratio which <3,5 is associated with elevated atherosclerosis risk only in women; 3) integrated biomarker BIO represented the combination of individual visual and biochemical variables and permitted to discriminate patients from those with no coronary atherosclerosis or having subclinical or severe atherosclerotic lesions.Conclusion.Proposed multimarker diagnostic panel could be regarded as novel potential biomarkers of coronary atherosclerosis risk and severity, however validation of these markers is necessary. The problem of cardiovascular risk stratification and further prevention activities should be solved with the search for novel markers and combinations of markers.

Adiponectin, endothelin and nitric oxide (NO) are major regulators of vascular function. An imbalance of vasoactive factors contributes to the onset and progression of atherosclerosis. Various single nucleotide polymorphisms (SNPs) are considered to be risk factors for coronary heart disease. However, the molecular mechanisms of their associations with the components of endothelial dysfunction are poorly understood. In the present study, rs17366743, rs17300539, rs266729, rs182052 and rs2241766 SNPs of the adiponectin (ADIPOQ) gene and rs2070699, rs1800542 and rs1800543 SNPs of the endothelin-1 (EDN1) gene were genotyped in 477 patients with coronary heart disease who were subjected to coronary angiography, in order to determine the presence or absence of coronary atherosclerosis. The serum levels of adiponectin, endothelin and stable metabolites of NO, (nitrate and nitrite NOx), were assayed and their associations with the SNP genotypes and coronary lesions were calculated. The results indicated that rs17366743 of the ADIPOQ gene and rs2070699 and rs1800543 of the EDN1 gene were associated with the levels of NOx in women, which in turn was associated with cardiovascular mortality. In men, rs182052 and rs266729 of the ADIPOQ gene were associated with adiponectin levels, whereas rs17366743 of the ADIPOQ gene was associated with endothelin levels. Additionally, these SNPs were indirectly associated with the prevalence of coronary lesions in men. Therefore, the tested SNPs can be considered potential risk factors that lead to imbalance of vasoactive mediators in a gender-specific manner and contribute to the development of clinical manifestations of atherosclerosis.
