Relevance of internal time and circadian robustness for cancer patients

Abstract: BackgroundAdequate circadian timing of cancer treatment schedules (chronotherapy) can enhance tolerance and efficacy several-fold in experimental and clinical situations. However, the optimal timing varies according to sex, genetic background and lifestyle. Here, we compute the individual phase of the Circadian Timing System to decipher the internal timing of each patient and find the optimal treatment timing.MethodsTwenty-four patients (11 male; 13 female), aged 36 to 77 years, with advanced or metastatic gas… Show more

“…Thus, through temporal transcriptional control, the clock is able to regulate the timing of the cell cycle. Such circadian gating of cell cycle has practical implication for timing of surgery, radiation therapy, and administration of cell cycle inhibitor drugs for cancer treatments (Dallmann et al, 2016; Levi and Schibler, 2007; Ortiz-Tudela et al, 2016). The clock also regulates inflammation, which is a common cause of metabolic disorders and cancer.…”

“…Imagine one could have surgeons work at their “best time” and most restored, with also the patient being under surgery when her/his body is most restored. It has successfully been shown that appropriately timed medication and chemotherapy enhance the effectiveness of a treatment both in experimental and clinical situations (Halberg et al, 1980; Hrushesky, 1990; Ortiz-Tudela et al, 2016; Truong et al, 2016). Of course, most hospital routines would need to be restructured to permit such flexibility.…”

“…To establish a rational chronotherapeutic strategy for choosing the appropriate dosing time, it is compulsory to identify the CTS status and predict optimal treatment timing in cancer patients. With respect to this translational point, Ortiz-Tudela and co-workers proposed monitoring of temperature, activity and position of metastatic gastrointestinal cancer patients by employing inner wrist surface Temperature, arm Activity and Position (TAP) [ 195 ]. Pursuing of circadian biomarkers seems smart approach but clinical validation is necessary in that inter- and intra-individual changes for these parameters.…”
