Primary adenocarcinoma of the seminal vesicles. A review of the literature

Abstract: Primary adenocarcinoma of the seminal vesicles (SV) are extremely rare and approximately only 60 cases have been reported in the literature. Due to the lack of specific symptoms the patients often present in an advanced stage of their disease. The only clinical examination that can indicate the presence of a neoplasm in the SVs is the digital rectal examination (DRE). Serum prostatic specific antigen (PSA) and prostate specific acid phosphatase (PAP) are usually normal in patients with primary adenocarcinoma o… Show more

“…DRE, CT, and MRI are useful for diagnosis of adenocarcinoma of the SV. DRE commonly reveals a painless, palpable mass (~70 % of patients) [1,4,5]. While the serum PSA level is normal, CA125 is a useful diagnostic and therapeutic marker for adenocarcinoma of the SV, initially showing an elevated level but falling if the patient responds to treatment [2].…”

“…The tumour was a poorly differentiated adenocarcinoma and the immunostaining pattern was showing negative staining for PSA, PSAP, CK 20 and CEA along with a strong positivity for CK7 corresponding with the immunostaining pattern observed in previously reported cases 2–11. Other histological types that have been reported include sarcomas, squamous cell carcinomas, lymphomas, germ cell tumours, gastrointestinal stromal tumor and neuroendocrine tumours 11. Benign tumours are even rarer and include cystic dysplasia, fibroepithelial tumours, leiomyoma, cystadenoma and schwannoma 11…”

“…On IHC, these atypical cells lining the papillary structures were positive for CK7, CA125 and PAX-8, and negative for PSA, AMACR, CK20, CDX-2, p63, GATA3, WT1, and calretinin. The Ki-67 index is about 30%-40% in these cells ( Figure 2 PSVA is an extremely rare tumour, with about 100 cases reported in the literature (Campobasso, Fornia, Ferretti, Maestroni, & Cortellini, 2012;Egevad, Ehrnström, Håkansson, & Grabe, 2007;Katafigiotis et al, 2016;Montironi et al, 2014;Ormsby, Haskell, Jones, & Goldblum, 2000;Oxley, Brett, Gillatt, & Burton, 1999;Posenato et al, 2017;Terrisse et al, 2017;Thiel & Effert, 2002;Yin & Jiang, 2018).
