Background: Matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) is a rapid and reliable method for microorganism identification. Herein, we compared the performance of the recently developed ASTA MicroIDSys (ASTA, South Korea) with that of the Bruker Biotyper (Bruker Daltonics, Germany) for identifying various Acinetobacter species. Methods: A total of 207 specimens comprising 22 Acinetobacter type or reference strains and 185 clinical isolates previously identified using molecular methods were tested per the manufacturers' recommendation, and the obtained results were compared. Results: The overall correct identification rates at the species level using the Bruker Biotyper and the ASTA MicroIDSys systems were significantly different (P <0.001) at 89.4% (185/207) and 96.6% (200/207), respectively. The correct identification rates within the Acinetobacter baumannii (Ab) group were similar (P =0.094) at 94.9% (166/175) and 97.1% (170/175), respectively. However, the correct identification rates within the non-Ab group were significantly different (P <0.05), at 59.4% (19/32) and 93.8% (30/32), respectively. When the twelve strains were excluded as the species were absent from the Bruker database, the overall identification results did not differ significantly (P =0.289). Conclusions: Both instruments were suitable for identifying commonly isolated Acinetobacter species in clinical microbiology laboratories, showing equivalent performance for the Ab group, while the ASTA MicroIDSys identified the non-Ab group with higher accuracy than the Bruker Biotyper.

“…Προσβάλλει συχνότερα ασθενείς που λαμβάνουν αντιβιοτικά ευρέος φάσματος, ασθενείς που έχουν υποβληθεί σε χειρουργική επέμβαση και ασθενείς που είναι συνδεδεμένοι με αναπνευστήρα, προκαλώντας βαριές συστηματικές λοιμώξεις, όπως είναι οι λοιμώξεις του κατώτερου αναπνευστικού και ιδιαίτερα, βακτηριαιμίες, λοιμώξεις του ουροποιητικού, και δευτερογενή μηνιγγίτιδα (Levin etal., 2003), (Ardoino et al, 2016). Η παρούσα διατριβή είχε ως στόχο την καταγραφή του ολικού μικροβιακού φορτίου και την ταυτόχρονη μελέτη ευαισθησίας στις αντιμικροβιακές ουσίες των συχνότερων Gram αρνητικών και Gram θετικών μικροοργανισμών που απομονώνονται από το περιβάλλον του νοσοκομείου. Το υλικό της έρευνας αποτέλεσαν καλλιέργειες οι οποίες απομονώθηκαν από τα δείγματα του άψυχου (αέρας, επιφάνειες) και έμψυχου περιβάλλοντος (χέρια του ιατρικού και νοσηλευτικού προσωπικού), σε τέσσερις κλινικές του Πανεπιστημιακού Γενικού Νοσοκομείου Έβρου. Συνολικά, ελήφθησαν 402 δείγματα, 125 δείγματα από το έμψυχο περιβάλλον και 277 από το άψυχο περιβάλλον. Ο έλεγχος ευαισθησίας στις αντιμικροβιακές ουσίες υλοποιήθηκε με δύο μεθόδους: α) με τον αυτοματοποιημένο αναλυτή Vitek 2 (Biomerieux) για την εκτίμηση ελάχιστης ανασταλτικής συγκέντρωσης ΜΙC με τη χρήση ειδικών καρτών και β) με τη μέθοδο E-test. Η στατιστική ανάλυση των αποτελεσμάτων έγινε με τη βοήθεια του στατιστικού πακέτου IBM SPSS v. 17.0.Από το έμψυχο περιβάλλον απομονώθηκαν 94 στελέχη (75,2%), εκ των οποίων τα 37 χαρακτηρίστηκαν πολυανθεκτικά (39,4%). Το ποσοστό αυτό αντιστοιχεί στο 29,6% επί του συνόλου των δειγμάτων του έμψυχου περιβάλλοντος.Από το άψυχο περιβάλλον απομονώθηκαν 106 στελέχη (38,3%) εκ των οποίων τα 24, χαρακτηρίστηκαν πολυανθεκτικά, (22,6%). Το ποσοστό αυτό αντιστοιχεί στο 8,7% επί του συνόλου των δειγμάτων του άψυχου περιβάλλοντοςΗ γενετική ανάλυση των περιβαλλοντικών στελεχών οδήγησε στα εξής συμπεράσματα αναφορικά με τη μοριακή επιδημιολογία τους. Τα επιδημιολογικά στελέχη έχουν το ίδιο γενετικό αποτύπωμα, ενώ τα σποραδικά εμφανιζόμενα και τα μη σχετιζόμενα επιδημιολογικά στελέχη παρουσιάζουν σταθερά γενετική ετερογένεια.
