The value of routine bone marrow biopsy in patients with diffuse large B-cell lymphoma staged with PET/CT: a Danish-Canadian study

Abstract: In patients with DLBCL, staging PET/CT can miss BM involvement with concordant DLBCL (less common) or discordant iNHL (more common). Routine BMB does not add relevant diagnostic or prognostic value over PET/CT alone in the majority of patients with DLBCL.

“…One consistent criticism regarding previous works addressing only diagnostic performance, including ours, was that independently of accuracy, PET/CT did not impact survival [3,5,19]. However, in our view, this criticism was based on conflicting data as a number of papers have shown that lack of prognostic value [6,7], whereas others did find it [8,9]. Interestingly, the largest series study in the setting of DLBCL so far sustains the abandon of the routine use of BMB based on the impact of PET/ CT on both OS and PFS, and the unlikelihood of BMB upstaging or driving clinically significant changes [9].…”

The inconsistency of data in the current literature can be explained by some methodological issues.…”

“…In diffuse large B-cell lymphoma, diffuse uptake is more likely to indicate reactive change than lymphomatous involvement in the bone marrow compartment. However, when diffuse uptake is due to bone marrow involvement, the uptake usually reflects diffuse low-volume, and sometimes discordant, cellular infiltration (35)(36)(37), which probably has less of an impact on prognosis (38,39) than areas of tumor bulk. In follicular lymphoma, diffuse cellular infiltration of the bone marrow is commonly missed by 18 F-FDG PET (40).…”

“…In T‐cell lymphomas, FDG‐PET/CT detects additional disease sites in 50% of the patients, but the clinical relevance of additional FDG‐PET/CT findings is uncertain as many patients have advanced stage already by conventional staging . FDG‐PET/CT is less sensitive for detection of bone marrow involvement in aggressive NHL (71–86% for DLBCL and <30% for peripheral T‐cell lymphoma [PTCL]) and diffuse bone marrow FDG‐uptake is seen more often in the setting of positive trephine biopsy suggesting that its interpretation depends on histology . Discordant bone marrow involvement by indolent lymphoma is also diagnosed in ~7% of patients with DLBCL and can be relevant for survivorship care .…”
