Objective: This study investigated the safety and efficacy of remote programming of cochlear implants. Study Design: Single-subject design Setting: Four North American clinical sites Patients: Forty cochlear implant recipients aged 12 years or older Intervention: Subjects had their cochlear implants programmed at a location that was remote from their audiologist using telecommunication with and without the support of a facilitator. Main Outcome Measures: Consonant-Nucleus-Consonant (CNC) word scores and the Speech, Spatial, and Qualities of Hearing Scale-C (SSQ-C) were compared using the subject's in-office MAP (program) and MAPs programmed remotely with and without the assistance of a facilitator. Additional subjective preference data were gathered from subjects and audiologists via questionnaires. Results: MAPs programmed via the three different models did not yield significantly different group mean CNC word scores. No device/procedure-related adverse events occurred. SSQ-C questionnaire results indicated that recipients received similar subjective benefit from familiar in-office, remote-facilitated, and remote-unassisted MAPs. Conclusions: Remote programming is an effective means of cochlear implant service delivery. The practice was approved by the FDA on November 17, 2017 supported by the results of this study.

Introdução: A teleconsulta pode ser benéfica em diversos aspectos: social, econômico, terapêutico e normativo. Na audiologia, a realização da teleconsulta vem sendo bastante estudada em alguns serviços, principalmente, na programação remota de usuários de Implante Coclear. Objetivo: Investigar a aplicabilidade, vantagens e desvantagens da teleconsulta em audiologia para os usuários de Implante Coclear. Método: Trata-se de pesquisa de revisão sistemática em que foram realizadas buscas nas bases de dados sem limitação de ano. Foram selecionados e incluídos, somente estudos de intervenção não randomizados, estudos descritivos e estudo de caso. Para leitura na íntegra, foram avaliados os procedimentos de aplicabilidade da teleconsulta, descrição dos pontos positivos pelos pacientes, profissional ou entre profissional e paciente, além da descrição de suas desvantagens. Resultados: Foram incluídos quatorze estudos, da língua inglesa e português do Brasil. Foram encontrados estudos de seguimento remoto nos seguintes casos: mapeamento, testes de fala (mensurações de níveis T e C), técnicas e configuração de plataformas remotas e orientações fonoaudiólogicas. Conclusão: Há diversas maneiras que possibilitam o sucesso do Implante Coclear na teleconsulta como mapeamento, orientações e terapia. Os usuários referiram benefícios e satisfação como redução de custos entre outros. Os facilitadores referiram contribuição em sua atuação profissional como reabilitador, e especialistas sentiram-se seguros ao realizarem os procedimentos de mapeamento. As desvantagens foram: atraso na conexão, atraso do estímulo e, consequentemente, atraso nas sessões, longo tempo de reverberação do estímulo auditivo.

Objectives: The telephone use is correlated to the quality of life, and is one of the greatest expectations of the patients who candidate for cochlear implantation. The objective of this work was to evaluate the benefit of a progressive and intensive training including 18 sessions, directly conducted on telephone, in cochlear implant recipients. Material and Methods: Nine cochlear-implanted adults underwent with a Speech and Language Pathologist, remote training specifically focused on the telephone use . They were evaluated before and after this training. Evaluation was comprised with monosyllabic words (Lafon), and sentences (MBAA) in quiet, cocktail-party noisy environment, and via phone.The telephone use (ad hoc surveys, number of calls), and the quality of life (Ersa, Aphab)were aseessed. Results:Before training, the repetition of monosyllabic words was lesser by telephone than in direct voice (and 64 % ± 5.7 vs 26 % ± 5.3,mean ± SEM, p <0.05). After the 6-weeks training, the "note taking" task of a telephone message was improved (85.0 ± 3.7 vs 50.0 ± 9.0 out of 100, p<0.001). Daily use of phone self-evaluated (57.0 ± 4.3 vs 29 ± 5.4 out of 100, p <0.0001) and the number of calls made the week before the assessment also improved (0.0 ± 0.0 vs 11.0 ± 3.0, p <0.0001). Conclusions:A progressive and intensive training program delivered over the telephone, improved the use of the telephone in daily life of cochlear-implanted adults.
