Neonatal sepsis is mediated by maternal fever in labour epidural analgesia

Abstract: Women delivering with EA (EA group) were matched on parity with 453 women with deliveries without EA (non-EA group). Significantly more neonates born in the EA-group had fever ≥ 38.0°C (11.6% vs 1.8%, p < 0.001) at birth. The overall incidence of neonatal sepsis, based on clinical symptoms and defined as proven (by a positive blood culture) or suspected (no positive blood culture), was significantly higher in the EA group (6.0% vs 2.2%; p = 0.002), but the incidence of proven neonatal sepsis alone was not (0.4… Show more

“…Maternal complications include the need for cesarean or instrumental delivery, while a high proportion of these are indicated by labor dystocia or suspected fetal distress [2][3][4][5] . Fetal complications include neonatal intensive care unit (NICU) admissions 2 , early onset neonatal sepsis 6 , low Apgar scores 2,7 , hypotonia, assisted ventilation, early-onset seizures 7 , neonatal encephalopathy 8 , hyaline membrane disease, and perinatal mortality 9 , as well as cerebral palsy in the long term 10,11 .…”

“…With epidurals, there are increased risks for the need for urinary catheterization and instrumental birth. Epidurals are also associated with the development of maternal fever during labor, which can lead to separation of mother and baby after birth and testing and antibiotic administration for the baby (Greenwell et al, 2012;Leighton & Halpern, 2002;Lieberman & O'Donoghue, 2002;Wassen et al, 2014).…”
