Objective:To analyze if the oral health conditions in children and adolescents are associated with hemophilia (PROSPERO-42020168192). Material and Methods: The search strategy was performed in PubMed, Scopus, Lilacs/BBO, Web of Science, Cochrane, and Grey literature databases. Two independent researchers assessed the risk of bias in these studies by the Newcastle-Ottawa Scale. For the meta-analysis, the clinical conditions data were extracted as numerical variables according to their indexes, such as dental caries experience (dmft/DMFT), gingival condition (Modified Gingival Index -IGM), and oral hygiene (Plaque Index -PI). The quality of the evidence of the meta-analysis was evaluated by the GRADE tool (GRADEproGDT). Results: From a total of 431 studies, 27 were included, and 10 were included in the meta-analysis. The studies presented a moderate risk of bias, ranging from 2 to 7 points. The dental caries experience in primary (-0.62; CI95%: -1.68-0.43) and permanent dentitions (-0.05; CI95%: -0.69-0.59), gingival condition (-0.12; CI95%: -0.27-0.03), and oral hygiene (0.36; CI95%: -0.06-0.77) did not differ between the groups. Conclusion: Based on studies with very weak evidence, there were no differences in the oral health conditions of children and adolescents with and without hemophilia.

Background For patients with hereditary coagulopathies such as hemophilia, the most common dental problem is spontaneous oral bleeding caused by daily oral hygiene habits like tooth brushing. Because of fear of bleeding, children with hemophilia often skip routine oral hygiene procedures, which can lead to dental caries and periodontal diseases. This case control study aimed to evaluate the oral hygiene awareness and hygiene practices of children with hemophilia from 6 to 18 years of age in Bulgaria. Material/Methods This study was conducted over 4 years and included 31 patients (children). A sociological approach was used, with direct individual questionnaires for self-assessment of dental habits. Statistical analyses were performed using SPSS. Results There was no significant difference between groups in methods of oral hygiene. The duration of oral hygiene procedures was 1 min (n=5, 16.13±6.61%) or 2 min (n=13, 41.94±8.86%), which was considered insufficient for proper oral hygiene. Children who had received oral hygiene advice by a dentist brushed their teeth significantly longer (n=9, 69.23±12.80%) than those who had not. There was a considerable difference between the reasons for the last dental visit between the groups of children (χ 2 =5.18, P <0.05). Conclusions Children with hemophilia have high awareness of oral hygiene methods; however, more attention should be focused on additional individual educational methods at the dental office. Frequent professional and self-assessment of the oral hygiene routine of children with hemophilia could identify the factors compromising dental status in this vulnerable population.

Purpose of Review This review aims to summarize the literature on inherited hemoglobin and coagulation disorders, focusing on sickle cell disease (SCD), thalassemia, hemophilia, and von Willebrand disease (vWD), and their impact on oral health. Clinical dental management of patients with these conditions is also reviewed. Recent Findings There is no evident association between caries and SCD. Despite limited available studies, there may be a possible association between caries and thalassemia, which could relate to salivary changes in immunoglobulin A and Streptococcus mutans reported in these patients. Controversial results on the caries experience of hemophilia and vWD patients are found in the literature, which are likely to reflect differences in the provided preventive dental care of these patients rather than a biological association. Current evidence does not support any association between periodontal disease and SCD. The literature on periodontal disease and thalassemia shows controversial results although some evidence suggests differences in the gingival inflammation of thalassemia patients with increased levels of inflammatory cytokines and matrix metalloproteinases, deserving further investigation. The available literature does not provide evidence of an increased risk for gingival bleeding or periodontal disease in patients with hemophilia or vWD. Dental management of these patients requires appropriate measures to prevent infection and excessive bleeding. The use of local hemostatic may often be sufficient; however, in severe bleeding disorders, systemic hemostatic or replacement therapy may be required, and procedures are recommended to be planned with the patient's hematologist. Summary Maintaining good oral health through preventive dentistry is important for patients with inherited hemoglobin and coagulation disorders as that may prevent the risk of infection and the need for invasive dental treatments that can result in bleeding complications.
