Retinal exsudative Gefäßerkrankungen im Kindesalter: Morbus Coats und familiär exsudative Vitreoretinopathie (FEVR)

Abstract: This article reviews the pathophysiology of retinal vascular disease with emphasis on Coats' disease and familial exudative vitreoretinopathy (FEVR). Both Coats' disease and FEVR demonstrate vascular abnormalities and associated exudation. Coats' disease manifests as teleangiectasia and aneurysms. Exudative subretinal lipid deposits can be extensive. Coats' disease is in 90 % unilateral and affects predominantly otherwise healthy young males. If the retina is attached, laser and cryocoagulation are the method … Show more

“…Many studies, however, have analysed Asian populations [1,[12][13][14] , focused on specific predisposing pathologies [12,15] or solely included rhegmatogenous RD [3,9,10] . Here, we present a large European case series of a unit with 20 years' experience in RD management that includes all types of RD in 95 eyes of 87 children below 18 years of age.…”

“…This fact underlines the importance of a thorough bilateral examination of the retinal periphery to identify fellow eyes at risk of RD. In particular cases, i.e., with a suspected or confirmed history of prematurity or retinal exudative diseases such as FEVR, a wide-field fluorescein angiography can help to evaluate the indication for prophylactic laser treatment of the fellow eye [15,23] .…”

“…The differential diagnosis of peripheral exudative retinal vascular lesions includes ROP, Coats' disease, familial exudative vitreoretinopathy (FEVR), and Norrie's disease (ND) [ 6 , 7 , 10 ]. Lipid exudates may develop in FEVR, but the cases are usually healthy term infants with a positive family history, associated with bilateral peripheral retinal avascularity seen temporally [ 10 ]. FEVR cannot be excluded based on these findings, because X-linked recessive inheritance and FEVR cases in premature infants have been reported [ 11 , 12 ].…”

“…Coats disease is a well-known ophthalmologic disease, first described more than a century ago by the British ophthalmologist George Coats 1 . It is an idiopathic vitreoretinal exudative condition that threatens visual function of pediatric patients, with a special predilection for the male gender and unilateral presentation 2 , 3 , 4 . Clinical appearance is characteristic as retinal telangiectasias with intraretinal or subretinal exudation, in the absence of appreciable vitreoretinal traction 4 , 5 .…”
