PurposeTo evaluate changes in clinical practice in the field of refractive surgery in Korea over the past 10 years.MethodsA survey consisting of 59 multiple-choice questions regarding the preferred types of refractive surgery, excimer laser machine, and presbyopia surgery was mailed to 742 members of the Korean Society of Cataract and Refractive Surgery in January 2016, and 50 members responded to the survey. These data were compared with the 2005 or 2007 survey results.ResultsThe majority of respondents were in their 40s (54%), and the average number of refractive surgeries performed in one month was 53. The most commonly used excimer laser machine was the VISX S4 in both 2005 (32%) and 2015 (25%); however, a greater variety of machines (EX500 [18%], Allegretto wave Eye-Q [13%], AMARIS 750 [10%]) were used in 2015. The preferred corneal refractive surgery in 2015 was surface ablation (40%), representing a significant increase in its popularity compared to 2005 (15%) (p < 0.001). The popularity of laser in situ keratomileusis (LASIK) surgery decreased to 20% in 2015 compared to 48% in 2005 (p < 0.001). Eighty percent of LASIK procedures in 2015 were performed using femtosecond laser. In 2015, surface ablation and phakic intraocular lens implantation were preferred for the treatment of myopia less than -8 diopters and more than -8 diopters, respectively. The proportion of respondents performing presbyopia surgery in 2015 (76%) was significantly increased from 2007 (30%) (p < 0.001).ConclusionsOver the past decade, the most commonly performed corneal refractive surgery has changed from LASIK to surface ablation, and there has been a significant increase in the popularity of presbyopia surgery.

Purpose: To investigate the current situation and future trends in the field of refractive surgery in Korea. Methods: Since 1995, the members of the Korean Society of Cataract and Refractive Surgery (KSCRS) have been annually surveyed on cataract and refractive surgery. The 2007 annual survey, consisting of 66 multiple-choice questions, was mailed in August 2008 to 336 KSCRS members. Fifty (14.9%) members answered. Current data were compared with those of previous annual survey and with those of the USA. Results: The respondents in their forties accounted for the highest percentage (38%) of the subjects, and VISX S4 was the most popularly used (32%). The use of IntraLase femtosecond laser (AMO, USA) had increased. LASIK still accounted for the highest percentage (48%) of refractive surgeries. LASIK was preferred for myopia less than -8D, and phakic IOL was preferred for myopia more than -8D. Almost all the respondents answered that most promising refractive surgery is phakic IOL and the respondents who preferred LASIK decreased. Conclusions: This survey summarized current trends and changes in cataract and refractive surgery in Korea.

Central Toxic Keratopathy After LASEK

Purpose: We report a case of transient corneal opacity and hyperopia after LASEK surgery. Case summary: A 21-year-old female visited our clinic complaining of decreased visual acuity in the left eye. The patient had undergone LASEK surgeryten days previously. Before LASEK surgery, the central corneal thickness of the left eye was 540 μm, and the refractive error was -2.00 Dsph=-0.75 Dcyl ×80A with an estimated ablation depth of 52.2 μm. At the time of visit (on the tenth day after surgery), the best corrected visual acuity (BCVA) was 0.07, the central corneal thickness was 404 μm, and the refractive error was +1.00D=+1.25D ×90. Slit-lamp biomicroscopy showed round central corneal haziness, but there were no signs of inflammation. At the third weeks after surgery, the central corneal thickness was 401 μm and the refractive error was +11.25D=-4.00D ×145. Slit-lamp biomicroscopy showed sustained round central corneal haze. Twenty-two weeks after surgery, the central corneal thickness was 445 μm and the refractive error was -0.75D=-1.25D ×180. The corneal opacity had disappeared.Conclusions: In case of transient corneal opacity, stromal loss and hyperopic change after LASEK surgery, central toxic keratopathy should be considered.
