[18F]FETO for adrenocortical PET imaging: a pilot study in healthy volunteers

Abstract: [(18)F]FETO is a valuable tracer for adrenocortical PET imaging, combining the longer half-life of( 18)F with a high 11beta-hydroxylase selectivity. In accordance with our findings in rats, FETO PET revealed very high accumulation in the adrenal glands in healthy volunteers.

“…Due to the binding of MTO to the 11β‐hydroxylase activity, the differential diagnosis of adrenocortical carcinomas versus other adrenal tumours such as phaeochromocytomas is possible using 11 C‐MTO also in the presence of extended necrosis (Hennings et al., 2006). 18 F‐FETO has also shown promise in imaging for adrenocortical imaging (Wadsak et al., 2006).…”

“…Several other 18 F-labelled metomidate analogues have also been proposed as adrenocortical imaging agents, but none have subsequently translated to clinical studies [55]. The only human study of 18 F-FETO, conducted in ten healthy volunteers, showed very high adrenocortical uptake and physiological distribution in the liver, renal calyces, gallbladder, stomach wall, and pancreas, with only faint uptake in the kidney and bowel [56]. Further clinical studies have, however, been lacking.…”

“…18 F-FE-TO/PET was shown to be significantly accumulated in the human adrenals of healthy volunteers [15]. Median SUVs in the adrenals were 6 times as high as that in the liver as a reference tissue [25]. Although these results indicated 11 C-ETO/PET and 18 F-FETO/PET are potentially useful for functional imaging of adrenocortical tumor, no additional study has been published in patients with adrenocortical diseases including PA.…”
