Background: Nowdays, the number of diabetic complications is still increasing each year. There are some factors that affect the early appearance of diabetic complications. By knowing these factors, doctors and patients will pay attention and prevent the early appearance. Therefore, diabetic patients will reduce the risk of death. Objectives: The study aimed todeterminethemost frequently occuringprofileofdiabeticmacroangiopathyandmicroangiopathy in peoplewithtype 2 diabetes mellitus. Method: Descriptive qualitativewith a cross-sectionaldesign. Results: Patients who are most commonly diagnosed type 2 diabetes mellituswithdiabeticcomplicationsare in theagegroup 46 -55 year (32%), male (50.6%), highschool educated (59.9%), privateemployees (36.6%), averageof HbA1c level is 8.68%, controlled HbA1c (54.5%),and durationoftype 2 diabetes mellituswithno data on duration (56.9%). The showncomplications are singlemicroangiopathy (30.6%), singlemacroangiopathy (22%), microangiopathyandmacroangiopathycombination (9.4%), multiplemicroangiopathies (2.7%) andmultiplemacroangiopathies (2.7%). The microangiopathycomplications are retinopathy (22.6%), nephropathy (22.1%), andneuropathicdiabetic (10.3%) whilethe mostmacroangiopathycomplications are coronaryheartdisease (19.2%), peripheralcirculatorycomplication (14.8%), andstroke (11%).

Abstrak Diabetes melitus merupakan penyakit gangguan metabolisme yang ditandai dengan meningkatnya kadar glukosa darah. Berdasarkan International Diabetes Federation (IDF), Indonesia merupakan urutan ke–7 negara yang memiliki kasus diabetes melitus terbesar. Penderita diabetes melitus ditandai dengan kadar glukosa darah pada waktu puasa ≥126 mg/dL dan 2 jam sesudah makan ≥200 mg/dL. Salah satu penyakit yang disebabkan karena komplikasi kronik mikrovaskuler pada penderita diabetes melitus adalah nefropati diabetik. Pemeriksaan ureum dan kreatinin merupakan cara paling sederhana untuk menilai status fungsional ginjal. Tujuan penelitian ini untuk mengetahui gambaran kadar ureum dan kreatinin pada penderita diabetes melitus tipe–2 di Rumah Sakit Santa Maria Pekanbaru periode Sempember–November 2018. Berdasarkan data pasien yang telah diperoleh, penderita diabetes melitus dengan kadar ureum >39 mg/dL (tinggi) paling banyak diderita oleh perempuan (9 orang), pada usia 55–64 tahun (4 orang), lama menderita 21–25 bulan (3 orang) dan dengan penyakit penyerta (7 orang). Kadar kreatinin >1,3 mg/dL (tinggi) paling banyak diderita oleh perempuan (7 orang), pada usia 55–64 tahun dan 65–74 tahun (6 orang), lama menderita 21–25 bulan (3 orang), dan dengan penyakit penyerta (6 orang). Berdasarkan penelitian ini dapat disimpulkan bahwa terjadi peningkatan kadar ureum dan kreatinin pada pasien diabetes melitus tipe–2 khususnya pada penderita perempuan. Background: Diabetes mellitus has strong correlation with end stage renal disease (ESRD) and responsible for 30-40% of all ESRD cases.This study is focused on assessing the diabetic nephropathy status in patients with type 2 diabetes. Glycated hemoglobin levels over therapeutic targets (>7%) had two times the risk of complications for diabetic nephropathy, ISN recommends the use of microalbuminuria and urinary albumin creatinine ratio (UACR) for early detection of diabetic nephropathy and for monitoring therapy. Objective: This study was conducted to prove the correlation between Glycated Albumin with microalbuminuria and UACR in type 2 diabetes. Methods: Cross sectional study was done in70diabetic type 2 patients who attended PROLANIS program in Primary Health Care from May to November 2018.Detailed medical history including the diabetes duration and relevant clinical examination like FBS, PPBS, HbA1c, urinary creatinineand urinary microalbumin were recorded in each patient.Significance is assessed at 5% level of significance. Results: This study obtained the mean age of the study population was 51.89 ± 6.78 years with female preponderance (51.1%).Mean FBS, PPBS, HbA1c, duration of diabetes, blood pressure, microalbuminuria and urinary creatinine were182.51 ± 74.63 mg/dL, 186.25±26.72 mg/dL, 8.8 ± 1.83%, 9.37±5.96 years, 138.44±14, 13/84.44±19.25 mmHg,30.32±3.2 mg/day and 1.33±0.64 mg/dl respectively.Microalbuminuria (r=0.91, p≤0.05) and UACR (r=0.67, p≤0.05) were positively associated with glycated hemoglobin. Conclusion: It can be concluded that microalbuminuria level and ACR increase in line with the worsening of glycosylated hemoglobin and diabetes duration.
