Residual C-peptide in patients with Type 1 diabetes and multiethnic backgrounds

Abstract: OBJECTIVE:To evaluate serum C-peptide in 88 patients from a multiethnic population with Type-1 diabetes and variable disease durations.METHOD:Eighty-eight patients with a mean disease duration of 8.1±7.6 years were included and underwent C-peptide measurement before and after glucagon stimulation. Chi-squared and Mann Whitney U-tests were used to compare the variables between groups (all two-tailed, α  = 0.05). Spearmańs correlation coefficient was used to test the association between the continuous variables.… Show more

“…Research in Germany and Italy also obtained similar results [31,32]. This is due to the duration of disease in this study subjects averaged still a decade so obtained low levels of C-peptide despite the increase of 25(OH)D 3 and this indicates that the progressive destruction of pancreatic β cells occur mainly in the first years of the emergence T1DM disease [38]. In addition, the onset that occurs at a young age also affects the levels of C-peptide [39].…”

“…It is likely that the improved glycaemic control that is associated with residual beta-cell function (5,(7)(8)(9)(10) results in reduced vascular damage, with hyperglycaemia acknowledged as the principal cause of microvascular complications (11). However, significant reductions in HbA1c and time spent in hyperglycaemia only occur at high beta-cell function (C-peptide ~200 pmol/L) (4,9,12), while even very low levels of beta-cell function may offer protection against diabetes-related complications (C-peptide >10 pmol/L) (5,7,13). It is unclear how beta-cell function may protect against complications independently of reducing hyperglycaemic induced damage.…”
