Association between Ankle-Brachial Index and Carotid Atherosclerotic Disease

Abstract: Patients with ABI < 0.9 showed a higher prevalence of carotid atherosclerosis. There was a negative correlation between ABI and IMT.

Ankle-Brachial Index as a Predictor of Mortality in Hemodialysis: A 5-Year Cohort Study

BackgroundAbnormal ankle-brachial index (ABI) has been found to be a strong predictor of mortality in some hemodialysis populations in studies with relatively short periods of follow-up, lower than 2 years.ObjectiveThis study aimed to assess the predictive value of abnormal ABI as a risk factor for death among patients on maintenance hemodialysis after a 5-year follow-up.MethodsA total of 478 patients on hemodialysis for at least 12 months were included in the study. ABI measurement was performed using a mercury column sphygmomanometer and portable Doppler. Patients were divided into 3 groups according to ABI (low: <0.9; normal: 0.9 to 1.3; and high: >1.3) and followed for a 60-month period.ResultsThe prevalence rates of low, normal and high ABI were 26.8%, 64.6% and 8.6%, respectively. The 5-year survival rate was lower in the groups with low ABI (44.1%, P<0.0001) and high ABI (60.8%, P= 0.025) than in the group with normal ABI (71.7%). Cox regression was used to evaluate the association between ABI and mortality, adjusting for potential confounders. Using normal ABI as reference, a low, but not a high ABI was found to be an independent risk factor for all-cause mortality (HR2.57; 95% CI, 1.84-3.57 and HR 1.62; 95% CI, 0.93-2.83, respectively).Conclusionslong-term survival rates of patients with either low or high ABI were lower than the one from those with normal ABI. However, after adjustment for potential confounders, only low ABI persisted as an independent risk factor for all-cause mortality among hemodialysis patients.

Background:Ultrasound measurement of carotid Intima-Media Thickness (IMT) is a suitable method to evaluate subclinical arteriosclerosis.Objectives:The current study aimed to present a new computerized algorithm to detect instantaneous changes of the IMT to Common Carotid Artery (CCA) of IMT in sequential ultrasound images by applying the maximum gradient and the dynamic programming.Patients and Methods:In a cross-sectional design, an examination was performed on thirty healthy human subjects with the mean age of 44 ± 6 years from April 2013 to June 2013 in Beheshti Hospital, Kashan, Iran. In all individuals, the instantaneous changes of the far wall IMT on the CCA were extracted. Local measurements of vessel intensity, intensity gradient, and boundary continuity were extracted for all of the sequential ultrasonic 2D-frames throughout three cardiac cycles. The Pearson correlation coefficients and Bland-Altman analysis were performed to assess the relationship and agreement between IMT measured by the proposed and conventional manual methods.Results:There was no significant difference between the proposed and manual methods with paired t-test analysis (in systole: 0.57 ± 0.10 vs. 0.56 ± 0.10 mm; P = 0.188 and in diastole: 0.63 ± 0.16 vs. 0.62 ± 0.10 mm; P = 0.122 for the manual and proposed methods, respectively). The Pearson correlation coefficients were r = 0.94 and r = 0.93 for IMTs and IMTd, respectively (both P < 0.001). Limit of agreements were narrow and considerable agreement was found between the two methods.Conclusions:The present study demonstrated that the proposed computerized analyzing method can provide accurate measurements of the IMT of the CCA in sequential 2D ultrasonic images.

Pendahuluan. Neuropati perifer diabetik (NPD) di kaki terjadi akibat komplikasi diabetes pada mikrovaskular yang akhirnya mengakibatkan kerusakan pada persarafan di kaki. Maka perlu diwaspadai proses aterosklerosis di tempat lain, baik pada pembuluh arteri makro maupun mikrovaskular di kaki. Penelitian ini dilakukan untuk dapat menilai hubungan derajat neuropati perifer diabetik yang dinilai dengan Toronto clinical scoring system (TCSS) dengan proses ateroskerosis di pembuluh darah kaki, baik yang makrovaskular dengan ankle brachial index (ABI) dan toe brachial index (TBI) maupun mikrovaskular dengan transcutaneus partial oxygen pressure (TcPO 2) pada pasien diabetes melitus (DM) tipe 2. Metode. Penelitian potong lintang dilakukan pada pasien DM tipe 2 dengan NPD dengan nilai TCSS >5 di Poliklinik Pelayanan Jantung Terpadu, Poliklinik Endokrin dan Metabolik, dan Poliklinik Ilmu Penyakit Dalam Umum RSCM. Data diperoleh dari wawancara, rekam medik, pemeriksaan ABI, TBI dan TcPO 2. Analisis bivariat terhadap masing-masing variabel dengan menggunakan uji spearman. Hasil. Sebanyak 36 subjek yang memenuhi kriteria pemilihan diikutkan dalam penelitian, rerata usia 62 tahun (SB 9,2) dengan 20 (55,6%) diantaranya perempuan dan median lama diabetes 12 tahun. Berdasarkan analisis bivariat dengan uji spearman penelitian ini mendapatkan korelasi negatif yang bermakna secara statistik dengan koefisien korelasi sedang antara derajat neuropati perifer diabetik yang dinilai dengan TCSS dengan ABI (r =-0,475, p = 0,003) dan TBI (r =-0,421, p = 0,010). Pada pemeriksaan TcPO 2 juga didapatkan korelasi negatif yang bermakna secara statistik dengan koefisien korelasi sedang (r =-0,399, p = 0,016). Simpulan. Terdapat korelasi negatif yang bermakna secara statistik antara derajat neuropati perifer diabetik dengan ABI, TBI, dan TcPO 2.
