The introduction of silicone in aesthetic procedures has raised concerns regarding the possible impacts of silicone on the body. Breast prostheses are still one of the most commonly performed plastic surgeries in Brazil and worldwide despite being linked to numerous complications. Additionally, the use of silicone has been correlated with autoimmune syndrome induced by adjuvant (ASIA), whereby an inflammatory response occurs due to the presence of a substance capable of increasing the immunogenicity of an antigen. This study addresses one such case of ASIA, also known as silicon disease, in order to assess its impact on the daily lives of patients who have undergone a breast prosthetic procedure. Fourteen articles relating to the patient’s case report were analyzed, collected from various platforms, including PUBMED, Scientific Electronic Library Online , Science Direct, and Google Academic. The patient had undergone breast augmentation in 2000. Two years later, she began experiencing symptoms of myalgia, arthralgia, and depression, implicating fibromyalgia. After removal of the prosthesis, the material was biopsied, and it was concluded that an immunological reaction had occurred due to the implant. Since little is known about ASIA, this study aims to increase the understanding of the syndrome and to assist future diagnostics.

Background: Over the past decades, evidence has accumulated that autoimmune symptoms can be triggered by exposure to environmental immunostimulatory factors that act as adjuvants in genetically susceptible individuals, with several unexplained symptoms. Adjuvant-induced autoimmune/autoinflammatory Syndrome (ASIA) was described in 2011 by Shoenfeld and Agmon-Levin and defined the diagnostic criteria. However, the association of this syndrome with silicone breast implants is controversial. Objective: To describe a case of ASIA syndrome in a silicone breast implant carrier and review of the literature. Clinical Case: JKT 36-year-old woman with luminal infiltrating ductal carcinoma; with negative extension studies, a conservative mastectomy of the skin and areola was performed with immediate reconstruction with an implant with a breast prosthesis and hormone therapy; Her follow-up was torpid with fatigue, pain in the arm, depression and sleep disturbances and affected breast skin, without infection. As she did not respond to conservative management, it was decided to remove the prosthesis, resection of the capsule and skin, due to the suspicion of the syndrome. From ASIA, as there was no involvement or persistence of breast cancer, with immediate improvement. Method: Bibliographic review of original articles written in English on cases of ASIA syndromes, associated with silicone breast implants from their description to the present. Results: Until 2019, 126 cases of ASIA syndrome due to silicone breast prostheses and implants were reported. Removal of the prosthesis improves symptoms. Conclusion: ASIA syndrome is recent, it is still considered an evolving concept, it is necessary to identify risk factors for the development of autoimmune phenomena in individuals who will receive some type of adjuvant to prevent cases of this syndrome.

Objetivo: Identificar sinais e sintomas experienciados por mulheres com síndrome autoimune induzida por adjuvantes (ASIA) devido ao uso de prótese mamária e os tratamentos realizados. Método: Estudo de campo de abordagem qualitativa realizado por meio de entrevistas online utilizando-se a técnica bola de neve. Incluíram-se 13 participantes. Resultados: A partir da análise dos dados, foram elencadas quatro categorias: conhecimento acerca da síndrome; sinais e sintomas; tratamento; e cuidados e implicações de Enfermagem. Identificaram-se mais de 120 sinais e sintomas, e o explante foi mencionado como tratamento definitivo por todas as entrevistadas. Os sinais e sintomas apresentados pelas participantes vão ao encontro do que é descrito pela literatura. Conclusão: Antes da descoberta da doença, as participantes realizaram tratamento com foco no alívio dos sintomas. Após o diagnóstico, todas as mulheres procederam com o explante.
